난청 아동 부모의 언어적 입력 양상에 관한 체계적 문헌고찰 및 메타분석

국문 초록

배경 및 목적: 부모의 언어적 입력은 아동의 언어 능력을 예측하는 요소로 오랜 시간 자리매김 해왔다. 본 연구는 체계적인 문헌고찰및 메타분석을 통해 난청 아동 부모의 양적 및 질적 언어입력 특성을 알아보고자 하였다. 방법: 체계적인 문헌고찰 방법에 따라 2001년부터 2022년 2월까지 국내외 학술지에 게재된 난청 아동 부모의 언어적 입력 양상에 관한 9편의 논문을 선정하여, 대상 논문의 질적평가와 메타분석을 실시하였다. 결과: 체계적 문헌고찰 결과, 대상 논문의 비뚤임 위험은 낮았고 부모-영유아 상호작용 평가에 참여한 대상자는 주로 36개월 이하의 아동과 건청 어머니였으며, 대부분의 난청 아동의 청력손실은 고도이상으로 인공와우이식 착용을 하였다. 메타분석 결과, 난청 영유아 부모 집단과 건청 영유아 부모 집단 간 총 발화 수는 유의한 차이가 없었으나, 총 낱말 수는 두 집단 간 유의한 차이가 있었다. 반면, 질적 언어입력은 다른 낱말 수 및 상위언어촉진전략 사용은 두 집단 간 유의한 차이가 나타났다. 논의 및 결론: 본 연구에서 난청 아동 부모와 건청 아동 부모는 다른 양상의 언어적 입력을 아동에게 제공하는 것을 알 수 있었다. 난청 영유아 부모발화의 특징은 건청 영유아 부모보다 더 짧은 발화 길이, 단순한 어휘의 반복, 더 적은 비율의 상위 언어촉진전략을 사용하는 것을 확인하였다. 본 연구 결과는 난청 자녀를 가진 건청 부모를 위한 적절한 부모코칭 전략이 개발되어야 함을 시사한다.

영문 초록

Objectives: Parental linguistic input has long been shown to predict children’s later language ability. This study aimed to characterize the quantity and quality of parental linguistic input of children who are deaf and hard-of-hearing (DHH) through systematic literature and meta-analysis. Methods: One thousand seven hundred sixty-one articles from 2001 to February 2022 investigating parental linguistic input in children with DHH were identified via database searches. Nine articles were identified that met the PICOS (Participants, Intervention, Comparison, Outcomes, and Study design) criteria. A systematic review and meta analysis of the literature were conducted. We used mixed effects models to estimate the overall mean effect size metric to address each research question. Results: As a result of the systematic literature review, the selected articles had a low risk of bias. In a considerable number of the presented meta-analytic articles, mothers and children aged 24-36 months participated. We did detect significant differences in the number of total words, the number of different words, and the proportion of the high-level facilitative language techniques from the parents’ utterances between the DHH and the typical hearing (TH) groups. On the other hand, there was not a significant difference in the number of total utterances between the two groups. Conclusion: This meta-analysis reports on different patterns of parental linguistic input between children with DHH and children with TH. We conclude with a discussion of the findings and consideration of the implications for research.
