팬데믹 속, 요양시설 거주노인들의 자기결정 지원: 돌봄종사자들과 함께 한 12주 동안의 실행연구 기록

국문 초록

본 연구는 코로나 팬데믹 하에서 오랜 격리로 어려움을 겪고 있는 노인들의 인간답고 주체적인 삶을 위한 노력에 초점을 두고, 돌봄 종사자들이 시설 거주 노인들의 ‘자기결정’을 실현하기 위해 어떤 지원전략들을 행하고 있는지, 어떤 장벽들이 존재하는지 탐색 및 분석하는 것을 목적으로 한다. 2개 기관 16명의 종사자를 대상으로 한 17회기의 계획-실행-반영을 반복하는 실행연구 과정을 통해 자료를 수집하였고, 지시적 내용분석을 통해 자료를 분석하였다. 본 연구는 선행연구와 팬데믹 하에서의 실천과정을 통해 시설노인 의 자기결정을 위한 구성요소로 7가지 실천의 틀을 제안하였는데 코로나 상황 속에서 ①외재적동기의 내재화, ②선호 반영과 가치에 대한 이해, ③신뢰에 기반한 친밀한 의사소통은 원활히 이루어진 것에 비해, ④영역, ⑤행위주체성, ⑥정보와 선택지 제공, ⑦충분한 시간의 전략은 잘 활용되지 않았음을 보고하였다. 정책적 제언으로 첫째, 거주노인의 자기 결정을 지원할 수 있는 구조를 마련하는 것, 둘째, 요양시설이 자기결정 지원을 위해 적극적인 시도를 할 수 있도록 우호적인 문화와 환경을 조성할 것 그리고 자기결정과 상충되는 가치가 존재할 때 기준선(border line)에 대해 적극적으로 논의할 수 있는 장(場)이 필요하다는 것을 강조하였다.

영문 초록

This study focuses on efforts for human rights and self-assertive lives of elderly people struggling with long quarantine under the COVID-19 pandemic. This paper aims to explore and analyze what support strategies care workers are carrying out and what barriers exist to help the elderly living in nursing homes make 'self-determination' well. Data were collected from 16 workers at two institutions through an Participatory Action Research(PAR) process consisting of 17 FGI sessions, and the data were analyzed through directed content analysis. This study proposed 7 frameworks of practice. In the last COVID-19 situation, three factors such as ①Internalization of Extrinsic Motivation, ② Understanding Preference & Value, ③Close communication based on trust were smoothly implemented, while strategies such as ④Areas, ⑤Causal Agent, ⑥Providing information and options, ⑦Providing Enough time were not well utilized. Based on the results of the study, we presented the following policy suggestions. First, it is necessary to establish a system that can support the self-determination of the elderly living. Second, a friendly culture and environment should be created so that nursing facilities can work more actively. Also it is emphasized that there is a need for a place to actively discuss the base-line when there is a value that conflicts with self-determination.
