만성질환 노인가계의 의료비 과부담 연구

국문 초록

본 연구는 만성질환을 경험하는 노인가구의 재난적 의료비 발생의 추이와 재난적 의료비 발생에 경제적으로 취약한 가구의 관련 영향요인들을 살펴보는 데 주 목적을 둔다. 최종적으로 가계소득 대비 20% 이상을 기준으로 의료비 과부담 여부를 결정하는 영향요인들을 살펴보았다. 연구결과, 만성질환 보유 개수가 0개에서 6개까지 노인성 만성질환 수가 1개 증가할 때마다 연평균 본인 의료비 부담금은 약 71만 3천원씩 증가하고 있음을 확인할 있었다. 이와는 반대로 가구원수 기준으로 균등화한 가계총소득은 만성질환 보유수가 증가할 때마다 11만 7천원씩 증가하는 것으로 나타나 부담금 증가분을 보충하는데 부족한 형편임을 알 수 있다. 즉, 노인성 만성질환 수의 감소가 개인들의 진료비 부담금 절감과 직접인 연관성이 있음을 발견할 수 있다. 분석대상 노인의 20%이상의 의료비 과부담에 영향을 주는 유의미한 요인을 관찰해 보면, 여성의 경우, 연령이 높을수록, 교육수준이 높을수록, 배우자가 있을 경우, 만성질환의 수가 많을수록, 암, 근골격, 순환기, 기타 만성질환을 보유하고 있을수록, 가계소득과 공적이전소득이 높을수록 재난적 의료비 부담을 할 확률이 높은 것으로 나타났다.

영문 초록

The purpose of this study is to investigate the trend of catastrophic burden of health expenses and characteristics of households that are vulnerable to that. In this empirical study, the author examined key influencing factors that determine out of pocket health expenses over 20% of household total income. As a result of this study, it was confirmed that when the number of chronic diseases increased from 0 to 6, the amount of medical expenses for the specific year was increased by approximately 713,000 korean won. On the contrary, the household gross income equalized by the number of households increased by KRW 117,000 every time the number of possession of chronic disease increased, indicating that it was insufficient to supplement the increase of the contribution amount. In other words, it is found that the decrease in the number of chronic diseases of the elderly is directly related to the reduction of the medical expenses contribution of the individual. More than 20% of the elderly who were analyzed showed significant factors influencing the health expenditure. The higher the age, the higher the education level, the spouse, the greater the number of chronic diseases, and other chronic diseases, the higher the household income and the public transfer income, the higher the probability of burdening disaster medical expenses is.
