국문 초록

고령화시대 국민건강보험의 변화는 피할 수 없다. 변화한 사회·경제 질서에 따라 국민건강보험도 변화해야지만 제 기능을 다 할 수 있기 때문이다. 고령화시대 국민건강 보험의 비중이 더욱 커지면서 국민건강보험에 대한 개혁의 요청은 더욱 강력해지고 있다. 우선 국민건강보험은 보험료 체계를 개선해야 할 것이다. 현재 국민건강보험료는 노인 노인에게 특히 지나치게 높다는 지적이 있다. 이러한 문제는 국민건강보험료 부과 체계가 가입자의 경제능력을 정확히 측정하지 못하는 데에서 비롯된다. 그리고 건강보험급여의 확대가 요구된다. 노인성 질환은 만성적인 것이 특징이다. 따라서 노인은 의료기관의 이용이 잦은데, 노인에게 현재의 의료서비스 비용은 본인일 부부담금만 부담한다 하더라도 지나치게 높은 편이다. 본인부담상한액이 규정되어 있지만, 그렇더라도 노인에게 의료부담이 높다는 비판은 피하기 어렵다. 또한 국민건강보험의 재정위기를 극복하기 위해 보험료를 조정하고, 재원조달방법을 개선해야 할 필요가 있다. 국민건강보험의 재원조달방법을 개선한다 하더라도 고령화 현상으로 인해 현 세대의 보험료 부담은 증가할 수밖에 없다. 하지만 현 세대의 희생을 일방적으로 강요해서는 안 되며, 반드시 세대 간의 합의가 이루어야 할 것이다. 마지막으로 의료수가가 적절히 산정되어야 한다. 의료서비스에 대한 적절한 보상이 이루어지지 않으면 결국 의료서비스의 질이 저하되기 때문이다.

영문 초록

The changes in National Health Service in the aging society are inevitable. It is because National Health Service can function to the fullest only if it follows the changes in social·economic structures. As National Health Service becomes more important in the aging society, the demand for its reform is getting much stronger. First of all, National Health Service needs to improve the insurance premium rating scheme. It’s been pointed out that senior health insurance tends to cost way more than the insurance plans for other age groups. This problem is attributed to the fact that National Health Service’s insurance premium rating scheme cannot accurately measure the subscribers’ economic competences. The national insurance benefits need to be expanded. The diseases common among the elderly are likely to be chronic. So they go to the medical institutions very often but the costs for the medical services provided are way too much for them even if they pay the deductibles only. The maximum deductibles are set, yet it seems difficult to avoid criticism that medical services cost way high for the elderly. In addition, insurance premiums need to be adjusted and financing methods need to be improved to overcome the financial crisis that National Health Service is currently facing. But even if the financing methods are improved, the increases in insurance premiums for the current generations are inevitable because of the aging society. Therefore, the current generations should not be forced to sacrifice and the agreement among generations must precede. Lastly, medical services need to be properly assessed. If medical services are not properly compensated, it would result in the decrease in the quality of medical services.
