DEA를 활용한 지방의료원의 운영 효율성 분석

국문 초록

본 연구는 33개 지방의료원을 대상으로 2012년부터 2016년까지의 운영 효율성을 측정하고, 효율성 영향 요인을 분석함으로써 효율성 개선을 위한 정책적, 전략적 시사점을 제공하는데 목적이 있다. 분석 모형은 DEA Excel Solver1.0, EnPAS1.0을 활용한 효율성 측정과 첨삭 Tobit 회귀분석을 활용한 효율성 영향 요인분석을 진행하였다. DEA 모형에 활용되는 투입 및 산출 요소를 AHP 기법인 Expert Choice11을 활용하여 선정하였다. DEA 효율성 분석결과, CRS의 경우 전체 기관의 연도별 효율성지수 추이는 전반적으로 상향세로 나타났다. 이 결과는 지방의료원의 조정의사 수ㆍ가동병상 수ㆍ의료비용 등 지방의료원의 투입변수는 매년 증가됨에 따라 조정환자 수ㆍㆍ당기순손익의 산출변수 역시 증가하여 운영적 측면에서 효율성이 존재했음을 확인하였다. 효율성 요인분석을 위한 Tobit 회귀분석 결과, 조정의사 수와 의료비용은 지방의료원의 효율성 지수에 유의미한 영향이 있는 것으로 분석되었다. 수익성과 효율성 차이 검증결과 흑자경영 그룹의 효율성 지수와 적자경영 그룹의 효율성 지수는 서로 차이가 없었다. 지방의료원별 효율성 및 비효율 요인을 파악하고 벤치마킹 포인트를 제시하였으며, 흑자와 적자경영 의료원과 효율성 간 관계를 규명함으로써 지방의료원이 공공성과 수익성을 동시에 고려해야 함을 제언하였다는데 의의가 있다.

영문 초록

This paper aims to provide policy and strategic implications for improving efficiency by measuring operational efficiency and analyzing factors affecting hospital efficiency in 33 regional public hospitals in South Korea from 2012 to 2016. Efficiency measurement analysis was conducted by using DEA Excel Solver1.0 and EnPAS1.0, while efficiency impact analysis used clipped Tobit regression. Inputs and outputs used in the DEA model were selected by using ExpertChoice11, an AHP technique. According to DEA Efficiency Analysis, the overall annual efficiency index trend of entire agency was elevated. This result also implies existence of operational efficiency, by increasing the number of adjustment patients and profit or loss of regional public hospitals, such as the number of medians controlled by regional public hospitals, the number of operating cases, and the cost of medical care. According to Tobit Regression for Efficiency Factor Analysis, the number of mediation physicians and the cost of medical care have a significant effect on the efficiency index of regional public hospitals. As a result of verification of differences in profitability and efficiency, there was no difference between the efficiency index of profit- and loss-making hospitals. In this study, we identified each regional hospital’s efficiency and inefficiency factors, and also presented benchmarking points as well. Also meaningful is the implication that local medical institutions should consider public and for-profit issues at the same time by demonstrating the relationship between profit- and loss-making hospitals.
