재가노인의 복지 및 보건·의료 서비스 조정 연계에 대한 연구

국문 초록

재가노인 장기요양 서비스의 보건·복지 연계와 노인 전문 코디네이션 활성화를 위한 제도적 노력의 필요성이 지속적으로 제기되고 있는 실정이다. 이에 본 연구는 프랑스 재가 장기요양 서비스 전달의 핵심적 요소이자 보건·복지 연계의 장(champ)으로서 기능하고 있는 개인별자립성수당(APA: Allocation Personnalisée d’Autonomie)의 사회의료팀 및 서비스 지원 계획과 노인 전문 코디네이션 기관인 CLIC(Centres Locaux d’Information et de la Coordiantion)에 관한 고찰을 통해 한국 재가 장기요양 정책 발전을 위한 새로운 방향 모색을 목적으로 하였다. 이를 위해 본 논문은 프랑스의 재가노인 정책에 대한 전반적 이해와, APA와 CLIC의 도입 배경 및 제도 시행 이후 변화된 지역 보건·사회복지 환경의 주요 측면을 살펴보았다. 그리고 재가노인의 APA 적용과 노인 전문 코디네이션 제도의 실용화 과정을 지역기반의 실제적 접근 방법으로 릴 사례를 중심으로 분석하였다. 그 결과 프랑스 APA와 CLIC의 운영 체계의 원리와 보건·복지 연계 기능 및 그 특징들을 도출해 낼 수 있었다. 이와 같이 APA와 CLIC의 적용 사례를 기반으로 한 본 연구는 한국의 재가 장기요양 정책 개선을 위한 지방 정부의 권한 확대, 서비스 지원 계획 수립 단계에서의 정보 교류와 유연성 확보, 노년학적 코디네이션에 대한 합의된 개념화와 전문적 관리 장치 구축을 그 시사점으로 제시하였다.

영문 초록

The objective of this study was to examine the characteristics of the coordination and delivery of elderly in-home long term care (LTC) services in France, and to recommend courses of action to improve the coordination of those services in Korea. Following an overview of French elderly home care policies, the conditions and changes in home care services after the introduction of the APA, an elderly LTC system, and CLIC, the gerontological mediation and linkage center for elderly community care, are researched. Next, the practical application of the APA, along with CLIC’s services and procedures, is analyzed through a case study approach focused on the city of Lille. On the basis of developments to the gerontological coordination system in France, three policy implications are suggested to improve the coordination of in-home elderly services in Korea: 1) the expansion of a local government’s authority and capacity, 2) the ensuring of information exchange and flexibility in the assessment and establishment of LTC plans, 3) a conceptual consensus regarding gerontological coordination and the need for mechanisms within the health and welfare association for the conception and professional management of this function.
