국문 초록

본 연구는 뇌졸중환자의 일상생활활동 수행력, 자아존중감과 부양자의 부양부담과의 관계를 살펴보고 부양자의 부양부담에 미치는 영향을 알아보고자 하였다. 뇌졸중을 진단받은 환자와 부양자 각각 113명을 대상으로 하였으며, 연구도구로는 뇌졸중 환자의 일상생활활동 수행력과 자아존중감 부양자의 부양부담을 알아보기 위해 한글판 수정바델지수(Korean-Version of Modified Barthel Index; K-MBI), 로젠버그 자아존중감척도(Rosenberg Self-Esteem Scale; RSES), 부양부담 측정도구를 사용하여 대상자가 직접 설문지에 답하도록 하였다. 자료처리는 빈도분석, 기술통계분석, 피어슨 상관관계와 단계별 다중회귀분석을 실시하였다. 연구결과 뇌졸중환자의 일상생활활동 수행력은 자아존중감, 부양자의 부양부담과 관계가 있었으며, 부양부담의 하위영역 중 시간-의존적, 발달단계상의, 신체적, 사회적, 경제적 부담과도 관계를 보였다. 자아존중감 역시 부양자의 부양부담과 관계가 있었으며 시간-의존적, 사회적, 정서적 부담과 관계를 보였다. 일상생활활동 수행력과 자아존중감 모두 부양자의 부양부담에 영향을 미치는 요인으로 나타났으며, 설명력은 33.5%였다. 부양자의 부양부담에 영향을 미치는 요인은 뇌졸중환자의 일상생활활동 수행력과 자아존중감이었음을 알 수 있었으며, 부양자의 부양부담을 줄이기 위해서는 신체적인 기능향상뿐만 아니라 환자의 내적인 심리에 대한 지지도 함께 이루어져야 할 것으로 생각된다.

영문 초록

This study was conducted to check the stroke patient's ADL, self-esteem and caregiving burden of caregivers and to investigate their relationship and effect on the caregiving burden of caregivers. The people in the study were 113 patients diagnosed with stroke and the same number of their caregivers and for the study tools, in order to check the ADL, self-esteem, caregiving burden of caregivers, each study participant filled out the questionaires using Korean version Modified Barthel Index(K-MBI), Self-Esteem Scale(RSES), Caregiver Burden Inventory. The data was processed through frequencies, descriptive statistics, Pearson correlation coefficient and stepwise multiple regression analysis. In the result of this study, statistically significant correlations were found between the patient's ADL and self-esteem, as well as time-dependence, developmental, physical, social and financial burden among synchronous sub areas of caregiving burden. Additionally self-esteem and time-dependence, social and emotional burden showed a statistically significant correlation. ADL and self-esteem were the factor affecting the caregiving burden of caregivers, with a 33.5% effect. We could conclude from the above that ADL and self-esteem of stroke patients are affecting factors on caregiving burden of caregivers. Consequently, it should be considered that not only the improvement of physical function but also the support for patient's internal psychology are both performed for reduce the caregiving burden of caregivers.
