Predictors of Functioning Recovery Trajectory Among Women with Psychiatric Disabilities

국문 초록

정신장애를 가지고 있는 어머니들은 정신질환이나 정신질환으로 인해 야기되는 부정적인 심리사회적 영향 때문에 양육에 어려움을 겪을 확률이 높다. 그러므로 정신장애가 있는 어머니들의 생활기능 수준을 양육과 관련해서 연구하는 것은 중요하다. 아직 한국에는 아이를 양육하는 여성에 대한 종단 데이터가 없기 때문에 본 연구는 유사한 미국의 종단데이터를 이용하여 정신장애가 있는 어머니들의 기능회복의 궤적과 궤적의 예측요인을 살펴보는 것을 목적으로 한다. 분석대상 은 1차 조사에서 333명의 아이를 양육하는 여성, 2차에서 이들 중 294명, 3차에서 1차 조사대상자중에서 291명을 추적 조사한 종단데이터를 사용하였다. 자료분석은 혼합효과모형을 이용하여 MIXREG를 사용하여 분석하였다. 분석결과는 정신장애를 가진 어머니들이 시간이 지남에 따라 생활기능 수준이 회복된다는 것을 보여주었다. 생활기능 수준 회복 궤적의 예측요인에 있어서는 긍정적인 환경이나 양육관련 변수들이 인구사회학적 변수나 정신질환 변수 보다가 생활기능 수준 회복 궤적과 더 밀접하게 관련이 있는 것으로 나타났다. 즉, 적은 스트레스, 높은 수준의 지지, 낮은 수준의 양육 스트레스, 아이와 좋은 관계를 가진 여성들의 기능회복 속도가 상대적으로 빨랐다. 즉, 정신장애가 있는 여성의 기능은 손상되어서 고착되어 있는 것이 아니라 시간이 지남에 따라 회복되며, 긍정적인 환경이나 양육은 이러한 기능회복 속도를 빠르게 해준다는 것을 의미한다. 이러한 결과는 환경이나 양육에 대한 개입이 정신장애가 있는 여성의 기능회복에 도움이 될 수 있다는 것을 보여준다. 연구결과의 함의에 대해서 실천과 후속연구 차원에서 논의하였다.

영문 초록

Mothers with psychiatric disabilities are likely to have difficulties in parenting due to impaired functioning levels caused by the nature of severe mental illness. Therefore, it is critical to examine functioning of mothers with psychiatric disabilities in relation to parenting. As there is no Korean research study available, this study aims to examine the trajectory of functioning and its predictive factors using a longitudinal U.S. data on child-rearing women. Sample consists of 333 child-rearing women with psychiatric disabilities at Wave 1, 294 at Wave 2, and 291 at Wave 3. Data was analyzed through mixed effect modeling using MIXREG software. Results showed that women presented increasing functioning recovery over time, indicating that their functioning is recovering with time. With regard to the predictors of the trajectory, context and parenting variables (e.g., stress, support, parenting stress, and relationship with children) were of more predictive of functioning recovery than demographic and mental illness variables. These findings suggest that interventions on context and parenting can be effective for functioning recovery among women with psychiatric disabilities. Implications of the findings were discussed in terms of practice and future research.
