비병변선 측두엽 간질의 예후 인자에 관한 연구

국문 초록

목 적 : 소아에서 국소발작은 전반적으로 전신발작에 비해 재발률이 높다. 또한 항경련제 투약 후 단기예후가 나쁜 경우에는 장기적으로 항경련제에 저항성을 나타내는 간질로 이행되는 경우가 많다. 이에 저자들은 비병변성 측두엽 간질 환자에서 항경련제 투약 후 단기 발작 소실 여부와 이에 영향을 미치는 임사지표 및 단기 발작 소실 여부가 장기 예후에 미치는 영향을 알아보고자 본 전향적 연구를 시행하였다. 방 법 : 1994년 6월부터 1997년 8월까지 비병변성 측두엽 간질 환자 32명을 대상으로 의무기록지를 통해 항경련제투약 1년째 6M-TR여부와 이에 영향을 미치는 임상지표와 6M-TR이 다음 1년간 지속적인 발작소실 유지에 영향을 미치는지를 통계 분석 하였다. 결 과 : 총 32명의 환아 중 진단시 평균 연령은 8.6±5.0세였으며 평균 이환기간은 28.0±10.4개월 이었다. 이 중6M-TR을 보인 환아는18(56.2%)명 이었다. 6TR에 영행을 미치는 예후 인자에 대해한 통계 분석상 발병 연령, 이환기간, 간질 발작의 유형, 뇌파 이상소견, 뇌 MRI 소견 등은 비병변셩 측두엽 간질의 예후에 영향을 미치지 않은 것으로 나타났다. 그러나 열성경련의 병력과 초기 유발인자로 주산기 가사의 유무는 단긱예후에 통계학적인 유의성은 없었으나 단기예후에 관계되는 추세를 보여 향후 더 많은 환자를 대상으로 조사해 볼 필요가 있을 것으로 사료된다. 6M-TR이 있었던 18명중 14명운 다음 1년간 지속적인 발작소실을 유지(77.8%)한 반면 6M-TR을 이루지 못한 14명중 4명만이 다음 1년간 지속적인 발작소실을 유지하여(28.6%) 통계학적으로 유의한 차이가 있었다(P=0.001). 결 론 : 본 연구에서 소아의 비병변성 측두엽 간질의 초기 단기예후는 비교적 양호하였으며 특히 6M-TR이 있었던 환자의 대부분은 다음 1년간 발작 소실을 계속 유지하는 경우가 높았으며 단기예후에 영향을 미치는 임상 양상을 비교 분석한 결과 특별히 영향을 미치는 임상 척도는 없었다. 따라서, 단기 발작 소실(6M-TR)은 소아에서 비병변성 측두엽 간질의 장기예후를 결정하는데 중요한 예후 인자로 사료된다.

영문 초록

Purpose : Temporal lobe epilepsy in infants and children have been the focus of many clinical investigations and observations. Several prognostic aspects of nonlesional temporal lobe epilepst(NLTLE) in childhood remain unclear or controversial. This study was aimed to evaluate the clinical characteristics of NLTLE influencing short-term remission and whether the short-term seizure outcome has any impact on long-term prognosis. Methods : The study was performed between June 1994 and August 1997. There were 32 newly-referred patients who had diagnosed of NLTLE from the data registry of Pediatric Epilepsy Clinic of Ajou University Medical Center. The patients identification was based on the careful review of hospital records, EEGs, and brain MRI. We have evaluated 6 months terminal remission rate(6M-TR) at one year of continuous antiepileptic drug(AED) treatment as short-term outcome. The predictive value of clinical parameters of NLTLE was comparatively analyzed between the patient who attained 6M-TR and who did not. To identify the long-term prognosis in NLTLE, we analyzed continuous seizure free rete during the next one year and compared between the patient who attained 6M-TR and who did not. Results : 1) Among total 32 NLTLE patients, 18(56.2%) patients attained 6M-TR at one year of AED treatment. 2) We cannot find any statistically significant clinical parameters influencing the short-term outcome between the two groups : age of onset(P=0.467), duration of illness(P=0.408), seizure type(P=0.725), abnormality of EEG(P=0.473), MRI findings(hippocampal sclerosis or temporal neocortical atrophy)(P=0.685). However, a previous history of perinatal asphyxia(P=0.367) and febrile seizure(P=0.253) were not statistically significant but those clinical parameters suggest clinical significance of influencing short-term outcome of NLTLE. 3) Patients with 6M-TR have showed the next one year remission in significantly higher proportion(77.8%) than those without 6M-TR(28.6%)(P=0.001). Conclusion : Early short-term outcome of NLTLE is relatively good and significant proportion of patients with early 6M-TR enters the next one year remission period. So we conclude that short-term remission of NLTLE may be an important determinant in predicting long-term prognosis.
