소아 간질 중첩증 환아의 임상적 고찰

국문 초록
목 적 : 간질 중첩증은 신속한 치료를 요하는 신경학적 응급 질환으로 때로는 심각한 신경학적 후유증이 발생할 수 있으므로 원인 질환과 예후에 영향을 미치는 인자들에 대한 정확한 이해가 필요하다. 이에 저자들은 소아 간질 중첩증의 원인과, 임상 양상, 신경학적 후유증의 발생 빈도를 알아보고자 이 연구를 시행하였다.
방 법 : 1992년 1월부터 1998년 12월까지 전북대학교 병원 소아과에서 간질 중첩으로 진단 받은 1개월에서 15세 사이의 환아 98명을 대상으로 나이에 따른 분포, 원인 질환에 따른 발생 빈도, 지속 시간에 따른 뇌파 및 CT나 MRI의 이상 소견의 빈도, 신경학적 후유증과 사망률을 분석하였다.
결 과 : 소아의 간질 중첩증의 대부분은 3세 미만에서 발생하였으며, 원인별로는 열성이 전체 98례중 43례(43.9%)로 가장 많았으며, 급성 증후성이 20례(20.4%), 특발성이 19례(19.4%), 원성 증후성이 13례(13.3%)였다. 경련 지속 시간은 1시간 이내가 55례(56.1%), 1시간 이상 2시간 이내가 23례(23.4%), 2시간 이상 24시간 이내가 17례(17.3%), 24시간 이상이 3례(3.1%)였다. 이중 열성 경련의 경우는 36례(83.7%)가 2시간 이내였다. 첫 경련 발작이 간질 중첩증인 경우가 53례(54.1%)였으며, 열성이 경우가 27례(62.8%)였다. 새로운 신경학적 후유증은 급성 증후성에서 10례, 특발성에서 5례 등 총 21(21.4%)에서 발생하였고, 사망의 경우에는 급성 증후성에서 3례, 진행성 뇌증에서 2례, 특발성과 원성 증후성에서 각각 1례로 전체적으로 7례(7.1%)였다.
결 론 : 간질 중첩증의 예후를 결정하는데 그 유발 원인과 간질의 지속 시간이 가장 중요한 요인 이었다. 소아의 간질 중첩증은 심각한 신경학적 후유증을 일으킬 수 있으므로 그 원인을 조기에 발견하고 효과적인 치료가 즉시 시행되어야 한다. 이에 간질 중첩증의 원인과 임상 양상, 예후에 영향을 미치는 인자들을 이해하여 앞으로 간질 중첩증에 대해 적극적으로 대처할 수 있는 능력을 함양 하는 것이 중요하리라고 생각된다.
영문 초록
Purpose : Status epilepticus(SE) is one of the most common neurologic emergencies in children requiring immediate aggressive intervention. Despite advances in treatment of this condition, SE is still associated with a significant morbidity and mortality. To evaluate etiology and neurologic outcome of SE is very important for improving the methods of evaluation and management of SE as well as decreasing morbidity and mortality.
Methods : We retrospectively reviewed the records of 98 children aged 1 month to 15 years who were admitted to our department of pediatrics from January, 1992 to December, 1998. We analyzed age distribution, etiology, seizure type, abnormalities of EEG and brain MRI, and neurologic outcome. We try to fing any coherence between neurologic sequelae and clinical factors.
Results : The incidence was high in toddler age, especially younger than 3 years old(67.4%). Generalized convulsive SE was the most common type(67.3%), and 54.1% of children with SE which was the first seizure episode. Major etiology of SE in young children younger than 3 years old were febrile convulsion(54.5%) and acute symptomatic causes(22.7%). Neurologic sequelae were left in 21.4% and the mortality rate was 7.1%. The neurologic complications were highly associated with the underlying causes and the seizure durations, especially over 2 hour seizures left in 75% complications. The mortality and neurologic sequelae were higher in acute symptomatic SE than other causes. Febrile SE had usually benign course, but in four in our cases showed neurologic sequale it shows 4 neurologic sequelae cases.
Conclusion : SE is a life-threatening neurologic condition and especially occurs in young children. The early detection of seizure etiology and aggressive management are the essential factors to decrease the mortality and morbidity rates.
