루게릭장애인 배우자 경험에 관한 사례연구

국문 초록

본 연구는 희귀난치성질환 중에서도 최중증이라고 할 수 있는 루게릭장애인 배우자 삶의 경험을 이해하고자 하는 질적 사례연구이다. 이를 위해 루게릭장애인의 발병 이후 중증화 되어가는 질환의 진행단계별로 배우자의 삶은 어떻게 변화 해 왔는지를 심층 분석하였다. 연구참여자는 10년 이상 호흡기 착용을 하고 있는 루게릭장애인 배우자 4명으로, 당사자중심 서비스 조직을 설립하려는 사람들로 의도적 표집(Purposive sample)을 하였다. 루게릭장애인 배우자 삶의 경험 의미는 총 17개의 범주와 ‘증상이 나타나는 신체’, ‘생명이 꺼져가는 신체’, ‘기계와 호흡하는 신체’, ‘가족의 배신과 사랑’4개의 주제로 분석되었다. 분석결과와 문헌검토를 종합하여, 루게릭장애인의 배우자 삶의 경험을 원가족 ‧ 배우자 ‧ 자녀 ‧ 사회와의 관계적 차원과 제도적 차원으로 나뉘어 논의하였다. 루게릭장애인 배우자 삶에 대한 심층적인 연구결과를 바탕으로 단계별 지원시스템의 구축과 활동지원제도의 보완이 필요하고, 루게릭장애인 가족을 위한 자조모임과 이들의 심리적 지원을 위한 패밀리피어카운슬러의 양성과 보급이 필요함을 제시하였다.

영문 초록

In this paper, I conducted a qualitative case study to understand the life experience of spouse caregiver of amyotrophic lateral sclerosis(ALS) or Lou Gherig’s Disease patients, which is the most incurable and fatal disease. In-depth analysis was used to examining the changes in the life of spouse caregivers accompanied by the progress of the disease. Four spouses of ALS patients who have been using a ventilator for 10 or more years was participated and were selected as purposive sample among people who intend to organize a　patient-oriented service organization. Meaning of the life experiences of spouse caregivers of ALS patients was classified into 17 categories　and 4 types of themes which can be divided into (1) body manifesting symptoms, (2) body losing hold on life, (3) body breathing through a machine, (4) family betrayal and love. Based on the analysis results and literature review, I discussed the experiences of the spouse caregivers at the relative and institutional level with direct family, spouse, children, society and institutional dimensions. As a result, I suggested the need to build a step-by-step support system, improve the activity support system, create a support group for families of ALS patients, and foster and provide family peer counselors as psychological support for them.
