지역사회정신보건사업의 주체

국문 초록

정신보건체계는 오랫동안 간호사, 의사, 임상심리사, 사회복시사 등이 포함되는 다학제적 팀에 의해 운영되어 왔다. 특히 이러한 다학제적 운영은 지역사회정신보건사업에서 포괄적인 치료를 담당하는데는 매우 중요하다. 전통적으로 병원입원을 중심으로 하는 임상정신의학에서 그 책임자는 대개 정신과의사였다. 하지만, 지역사회정신보건사업이 강조되고, 2000년 1월 12일 개정된 정신보건법에 따르면 앞으로는 반드시 그렇지만도 않다. 이러한 경향은 곧 여러 경우에 포괄적이고 생물-심리-사회적인 면에 가장 전문가인 정신과의사를 배제한 채 운영될 것이다. 이것은 중증 정신질환이 의학적 영역에 남아있는 한 부정적인 결과를 초래할 것이다. 지역사회정신보건체계는 가장 심각한 정신질환, 즉 정신분열병, 양극성 장애, 재발성 주요우울증 등을 앓고 있는 환자들의 치료 및 재활을 그 주요 목표로 하고 있다. 이 환자들의 적절한 치료 및 재활은 정신보건체계내 모든 기관에서 정신과의사의 적절한 리더쉽을 규정하여만 보장될 수 있다.

영문 초록

The psychiatric care has been delivered through the combined expertise of multidisciplinary teams that include, among others, nurses, psychiatrists, psychologists, and social workers. The multidisciplinary approach is vital to the provision of comprehensive care within community mental health setting. Traditionally, mental health hospital have been headed by psychiatrists. This will be no longer true for community mental health programs since the revision of Mental Health Law on Jan 12, 2000. As soon, the result of the trend will be that the comprehensive, biopsychosocial expertise brought by psychiatrists has been absent in many cases. This has negative consequences as serious mental illness are in the medical domain. The community mental health systems are responsible for the care and treatment of large numbers of persons with the most serious mental illnesses, eg. schizophrenia, bipolar disorder, recurrent major depression. The qualified care for these persons must be assured through defining appropriate leadership role for psychiatrist at all levels of the mental health care system.
