국문 초록

정신보건법 제정 이후 지역사회에 서울시 강남구와 경기도 수원시에 지역사회 정신보건센터가 설립되어 지역내 만성정신질환자를 위한 정신사회재활 서비스를 시작한 이래로, 중앙정부와 지방정부에 의해서 많은 정신보건센터가 설치되어 이제 전국에 44개소를 헤아리게 되었다. 이와 동시에 인구수가 적거나, 예산배정이 어렵거나, 전문인력이 부족해 센터가 설치되지 않은 지역에서는 방문보건 요원들에 의해 정신보건 서비스가 시행되고 있고, 사회복귀시설 또한 설치되어 많은 정신장애인들에게 사회복귀와 지지를 제공하여 입원을 줄이고, 기능을 호전시키는데 기여하고 있다. 그러나 수년간 갑자기 많은 지역사회 정신보건 시설과 서비스가 제공되면서 어떻게 효과적으로 재원이 분배되고 서비스가 제공되어야 하는가에 대한 논란이 있어왔다. 서비스의 공공성을 강조하는 견해를 가진 전문가 측에서는 보건소를 중심으로, 서비스의 접근성과 질을 강조하는 전문가 측에서는 민간참여형 센터를 중심으로, 또 기존 자원의 이용을 강조하는 쪽에서는 복지관을 중심으로 사업을 전개하자고 주장을 하고 있다. 이런 가운데 한정된 재원과 인력을 효과적으로 이용하자는 의견과 환자들과 가족이 요구하는 정신보건 서비스는 현실과 거리감이 있으므로 더욱 새로운 재원과 인력을 투자하자는 의견 또한 대립되고 있다. 본 논문에서는 이러한 지역사회 정신보건 사업의 다양한 모델들을 검토하고, 향후 우리가 채택할 수 있는 모델에 대한 의견을 제시하고자 한다.

영문 초록

After opening of community mental health center(CMHC) in Seoul and Kyonggi Province for psychosocial rehabilitation of the chronic mentally ills in the community, now there are 44 CMHCs nationwide established by the joint effort between central, local government and private sector. Because of small population, lack of budget, and lack of mental health professionals in the community, many public health offices(PHO) are providing mental health service primarily by the home visiting nurses. Residential facilities and vocational workshops are established to reduce recidivism and improve functioning of the chronic mentally ills. Rapid growing of these community mental health centers and programs called debate in the issues of financial resources distribution and service delivery system. Professionals who emphasized public aspect of these services advocate PHO-centered delivery of services. On the other hand, professionals who thought consumer-oriented mental health services accompanied with quality of care and easy accessibility urged CMHCs should be the center for community services for the integration of mental health services. Other professionals started services based on welfare facility to stress existing resource utilization in the community. Among these debates, an opinion that said utilization of limited financial and human resources is opposed to another that said new investment and reimbursement should be done. The author reviewed various models of community mental health delivery and suggest new model for the future.
