수정체유화술을 시행받는 환자의 혈압 변화

국문 초록

목적: 수정체유화술의 수술 전, 중 및 후 환자의 혈압변화를 조사하여 수정체유화술이 혈압에 미치는 영향을 알아보고 안전한 환자 관리에 도움이 되고자 한다. 대상과 방법: 점안마취 상태에서 정상 혈압을 가진 백내장 환자 60명과 순환기 내과에서 고혈압을 진단받은 환자 24명을 대상으로 입원 당시, 수술준비실 입실 직후, 감압계 착용 직후, 수술실 입실 직후, 수정체유화술 중, 수술을 마치고 병실로 온 직후의 혈압을 측정하여 전향적으로 비교 분석하였다. 결과: 정상 혈압 환자의 경우 입원당시 혈압과 수정체유화술 중 평균 혈압차이는 수축기혈압 26.15mmHg, 이완기혈압 7.26 mmHg으로 수술 중 통계적으로 의의 있는 상승소견을 보였다(p=0.001). 고혈압 환자의 경우 혈압약을 복용하였음에도 불구하고 입원당시의 혈압에 비해 수정체유화술 중 수축기 혈압이 19.4 mmHg, 이완기혈압은 4.08 mmH만큼 상승하였으며, 수축기 혈압 상승이 통계적으로 의의가 있었다(p=0.005). 고찰: 점안마취 상태에서 행해지는 수정체유화술 중 혈압에 대한 지속적인 감시가 필요하며, 고혈압 환자의 경우는 좀 더 엄격한 관리가 필요하다.

영문 초록

Purpose: To evaluate the influence of phacoemulsification on blood pressure for safer patient care. Methods: We conducted a prospective study in which we checked and compared the patients blood pressures at admission, on entering the preparation room right after reducer application, before and during phacoemulsification with topical anesthesia and on returning to admission room after operation. The patients comprised 60 normotensive and 24 hypertensive patients confirmed by cardiac specialists. Results: Significant elevation of blood pressure was noted during phacoemulsification in normotensive patients. The mean difference between the two occasions (blood pressures checked at admission and during phacoemulsification) was 26.15 mmHg in systole and 7.26 mmHg in diastole in normotensive patients (p=0.001). In hypertensive patients, despite on antihypertensive medications, systolic blood pressure significantly increased (19.4 mmHg) during phacoemulsification compared to the baseline value (p=0.005). The diastolic pressure increased by 4.08 mmHg. The elevation of systolic and diastolic blood pressure was 26.15 and 7.26 mmHg respectively in normotensive patients. Conclusions: Close monitoring of blood pressure is needed during phacoemulsification and special attention is required for hypertensive patients.
