밤가시에 찔린 후 수정체 전낭이 파열된 3예

국문 초록

목적: 밤가시에 찔린 후 수정체 전낭이 파열된 3예에서, 백내장 수술 후, 모두 좋은 시력을 얻었기에 이를 보고하고자 한다. 대상과 방법: 2명은 초진 시 경도의 전방내 염증 반응과 더불어, 밤가시가 박힌 채 방수 누출을 동반한 각막 천공 및 수정체 전낭 파열이 관찰되어 백내장 낭외적출술 후 PMMA 인공수정체를 삽입하였고, 각막에 박힌 밤가시도 제거하였다. 나머지 1명은 초진 시 외래에서 밤가시를 제거하였으며 전낭파열이 동반된 외상 백내장은 초음파수정체유화술과 접형 인공수정체삽입술을 시행하였다. 결과: 증례 1은 수술 직후 황반부종을 보이며 시력이 0.05로 불량하였으나, 수술 후 6개월에는 교정시력이 0.9로 회복되었다. 증례 2는 술 후 2개월째 시력이 0.8이었고 경과관찰 동안 전방 내 염증 소견은 보이지 않았다. 증례 3은 술 후 2개월째 시력이 1.0이었고, 전방 내 염증 등 다른 이상은 보이지 않았다. 결론: 밤가시에 찔린 후 수정체 전낭 파열로 진단받은 3예에 대해 신속히 적절한 조치를 취해 좋은 결과를 경험하였기에 이를 보고하는 바이다.

영문 초록

Purpose: To report three cases of anterior capsular rupture in lens caused by chestnut bur injury with good final visual acuity postoperatively. Methods: At the first visit, two patients showed corneal perforation with leakage caused by chestnut bur injuries and anterior capsular ruptures in lens. They were treated with removal of chestnut bur from cornea, extracapsular cataract extraction and intraocular lens implantation (PMMA). Third patient was treated with removal of chestnut bur at out patient office and then treated with phacoemulsification and foldable intraocular lens implantation. Results: Case 1 showed the macular edema so the visual acuity was poor as 20/300, but visual acuity was recovered into 20/22 at 6 months postoperatively. The visual acuity of case 2 was 20/25 at postoperative two months, and no abnormal finding was found during the follow-up period. The visual acuity of case 3 was 20/20 at two months and no abnormal findings was found postoperatively. Conclusions: We report three cases of anterior capsular ruptures in lens caused by chestnut bur injuries. The prompt and proper treatments brought about good results.
