통원 수술로 백내장 수술을 받는 환자군의 특성과 주술기 관리

국문 초록

목적: 통원으로 백내장 수술을 받는 환자군의 특징 및 위험 요소를 파악하여 주술기의 환자 관리에 유의할 사항을 파악하고자 하였다. 대상과 방법: 백내장 수술을 위하여 통원 수술실로 내원한 환자 337예를 전향적으로 관찰하고 의무기록을 분석하였다. 결과: 남자 141명, 여자 196명 총 337명의 환자가 연구에 참여하였으며, 평균 연령은 67.3세(20∼91)였다. 217명(64%)의 환자가 하나 이상의 질병을 가지고 있었으며 고혈압 환자가 180명(53%)으로 가장 많았고, 당뇨 환자가 106명(31%)이었다. 71명(21%)의 환자가 두 가지 이상의 질병을 가지고 있었다. 2%의 당뇨환자에서 수술 전에 저혈당이 관찰되었고, 고혈압 환자의 2.8%에서 수술 중에 지속되는 고혈압으로 혈압강하제가 투여되었다. 결론: 통원 수술로 백내장을 시행받는 환자들은 고령과 함께 당뇨, 고혈압 등 전신질환을 동반하는 경우가 많았다. 그러나 이러한 환자의 상태에 유념하여 수술 전과 수술 중에 혈압 및 혈당을 주기적으로 측정하는 등 주의를 기울인다면 백내장 수술은 통원 수술로 충분히 시행할 수 있으리라 생각한다.

영문 초록

Purpose: There is an increase in number of geriatric patients who have cataract surgery in ambulatory facility. Giving the geriatric population with its associated medical problems, perioperative complications may arise. This study was undertaken to understand patients characteristics and perioperative management of patients having ambulatory cataract surgery. Methods: A prospective study included 337 patients (196 female and 141 male patients) undergoing routine cataract surgery under monitored anesthetic care. A detailed questionnaire was completed by the anesthesia personnel. Results: The mean age of patients was 67.3 years old (range 20 to 91 years). Overall 217 patients (64%) had known medical conditions. Of those patients with medical conditions, 180 patients had hypertension, and 106 patients had diabetes mellitus. The fasting blood glucose level was 80 mg/dl or lower in two diabetic patients. Among patients with hypertension, five patients received antihypertensive medication during operation by anesthesiologist. Conclusions: Although significant medical conditions exist in patients having cataract surgery under monitored anesthetic care in ambulatory facility, most patients appear suitable for day case surgery provided they are well informed and carefully managed.
