남북전쟁 이전 미국 해군병원과 공중보건: 찰스턴과 뉴올리언스를 중심으로

국문 초록

본 연구는 남북전쟁 이전 미국 사우스캐롤라이나 주 찰스턴 시와 뉴올리언스 시의 해군병원 사례를 고찰 하여 해군병원이 미국 공중보건 역사에서 차지하는 의미를 찾고자 한다. 미국의 해군병원 의료체제는 미국 역사상 최초의 건강 관련한 국가적 노력의 결실이었고, 남북전쟁 이후 연방의무감(Surgeon General)이 관장하는 공중보건 국(Public Health Service)의 전신이었다. 1798년 연방 의회가 설립한 미국 해군병원 체제는 아프거나 장애가 있는 해군이나 선원들에게 의료 서비스를 제공하기 위해 설립 되었다. 미국 역사에서 연방권과 주권(州權, states’ rights) 의 갈등은 다양한 사안을 둘러싸고 주(州) 정부와 연방 정부, 주권(州權)과 연방권 사이의 갈등을 야기하였고, 그사안 중 하나가 바로 공중보건과 건강이었다. 이런 맥락에서 찰스턴과 뉴올리언스 해군병원의 역사는 연방 정부와 주(州) 정부, 또는 연방 정부와 민간 의료기관 사이의 갈등과 협상 과정이었다. 찰스턴 해군병원의 역사는 찰스턴시 당국과 연방 정부 간의 공중보건을 둘러싼 끊임없는 경쟁 관계를 보여주었다. 뉴올리언스 해군병원의 주도권은 독립적인 해군병원이 설립되기 전과 후에도 사설 병원 과의 협의를 통하여 이전되고 변경되었다.

영문 초록

This study examines the marine hospitals in Charleston, South Carolina and New Orleans, Louisiana, in order to reveal the floating nature of public health governance system in the United States during the antebellum period. The Marine Hospital Service was an organization of marine hospitals dedicated to the care of ill and disabled seamen. It was also the predecessor of the current U.S. Public Health Service. The marine hospitals, initially established by the federal government in 1798, was to provide medical care for sick or injured seamen. It was the first federal health-related measure, which authorized a tax-the deduction of twenty cents per month from the wages of the seamen. In US history, the conflict between the national and state governments over sovereignty caused a wide range of issues including that of public health. The histories of marine hospitals in New Orleans and Charleston were the story of the disputes or negotiations between the federal and state governments, or between the federal authority and private medical institutions over the issue of public health. The establishment of Charleston Marine Hospital showed unrelenting competition between municipal and federal authorities for the control of medical service over the sick seamen. In New Orleans, the focus of medical care for sick crews shifted from place to place according to contracts with private and public agencies.
