재미 한인노인의 장기요양 보호욕구와 부양체계

국문 초록

미국의 한인사회에도 노령인구가 증가하고 있으며 이에 따른 노인부양문제가 대두되고 있다. 본 연구는 시애틀의 노인아파트에 거주하는 저소득층 노인단독세대를 중심으로 재미 한인노인들의 장기요양보호욕구의 실태를 조사하고 이들의 사적, 공적 부양체계가 어떻게 구성되어 있으며, 그 한계가 무엇인지 규명하였다. 평균연령이 74세인 조사대상자들은 일상생활 수행능력(Activities of Daily Living/Instrumental Activities of Daily Living)에 있어서 아직 대부분이 타인의 도움을 필요로 하지 않았으나, 절반 이상이 건강 때문에 일상생활에 지장을 받는다고 하였고, 연령이 증가할수록 기능장애가 커짐으로써 장기요양보호의 욕구 또한 증가하는 것으로 나타났다. 한편 이들은 한 두명의 가족, 특히 자녀 중심을 사회적 지원망으로 구성된 사적 부양체계에 의존하여 도구적이거나 정서저인 지원을 받고 있었으며, 지원망과 비교적 가까운 거리에 살며 자주 전화연락을 취하고, 상호방문하며 친한 관계를 맺고 있었다. 한편 이러한 사회적 지원망의 효과나 만족도에 관해서는 대부분이 긍정적인 평가를 내리고 있었다. 이들은 또한 정부가 제공하는 소득보장, 의료, 주택, 가사보조 등 공적 부양체계에 대한 매우 높은 인지도를 보였으며, 이것을 폭넓게 활용하였다. 이와 같이 재미 한인노인들은 이민사회의 가족구조와 가치관의 변화에 적응하면서, 상호보완관례를 이루고 있는 사적부양체계와 공적부양체계를 통하여 기본적인 생활욕구 및 장기요양보호욕구를 충족시키고 있다고 하겠다.

영문 초록

The purpose of this research was to assess the long-term care needs and the support systems of the low income Korean-American older people who maintain separate living arrangement from their children. A structured questionnaire was used to interview a nonrandom sample of eighty-five Korean-American elders who were sixty years and older. The sample with an average age of 74 years had deteriorating health conditions, however, they had minor functional impairment, and the majority were able to maintain relatively independent daily activities of living. ‘This research also found the predominance of the small family-centered informal support networks in the lives of these elders. ‘The majority of the older Koreans relied on their spouses or children for most of their instrumental and expressive support needs. Moreover, formal support resources such as supplemental income, medical care, housing subsidy and paid-helpers also played significant roles in maintaining the basic standard of living and every day activities of these elders. As the Korean communities in the U.S. go through a rapid aging process, there is a need to assess the support systems and prepare for future challenges of long-term care.
