흡수성 인공삽입물을 이용한 안와골절정복술 후 발생한 지연성안와연조직염 1예

국문 초록

목적: 흡수성 삽입물을 사용하여 안와골절정복술을 시행한 후 지연성으로 발생한 골막하농양을 동반한 안와연조직염 증례를 경험하였기에 이에 대해 보고하고자 한다. 증례요약: 16세 남자 환자가 내원 1일 전부터 시작된 좌안 통증, 부종, 수직복시를 주소로 내원하였다. 환자는 20개월 전 좌안 안와하 벽골절로 MacroporeⓇ (Medtronic Inc., Minneapolis, MN, USA)를 이용한 안와골절정복술을 시행 받은 병력이 있었다. 시행한 안와전 산화단층촬영에서 삽입물은 명확히 관찰되지 않았으며, 좌측 안와하벽 골막하 낭성 종괴 및 주변의 침윤과 양측 상악굴염 및 벌집굴염 소견이 보였다. 3일간 전신적 항생제로 치료하였으나 호전이 없어 결막절개를 통한 안와접근법으로 골막하농양의 절개배농술을 시행하였다. 수술 시 삽입물은 거의 녹아 있었으나 작은 조각들이 남아 있었다. 배농하며 시행한 미생물검사에서 검출된 미생물은 없었다. 절개배농술 후 18일간 전신적 항생제를 지속하였고 임상증상은 호전되었다. 결론: 안와골절정복술을 시행 받은 환자에서 안구운동장애, 안와 부종 및 통증 등의 증상이 생긴 경우, 흡수성 안와삽입물을 이용하였더라도 지연성 안와연조직염을 의심하여야 하며, 영상검사를 통한 조기 진단 및 적절한 치료가 필요하다. <대한안과학회지 2016;57(7):1165-1169>

영문 초록

Purpose: To report a case of delayed orbital cellulitis with subperiosteal abscess after orbital floor fracture repair using an absorbable sheet implant (Macropore？？, Medtronic Inc., Minneapolis, MN, USA). Case summary: A 16-year-old male visited the oculoplastic clinic for left eye pain, lower eyelid swelling and vertical diplopia for 1 day. The patient had a history of inferior orbital wall fracture repair surgery using Macropore？？ 20 months prior. The orbital computed tomography scan showed a subperiosteal cystic mass with surrounding infiltration at the left orbital floor, and ethmoidal and maxillary sinusitis; however, sheet implant was not clearly observed. Despite systemic antibiotic treatment for 3 days, his clinical findings did not improve, thus we decided to drain the subperiosteal abscess through a transconjunctival approach. Intraoperatively, the Macropore？？ sheet was almost dissolved, but small pieces remained. The culture of drained contents showed no microorganisms. Systemic antibiotics were continued for 18 days after surgery, and clinical symptoms completely improved. Conclusions: Delayed orbital cellulitis should be considered in patients with extraocular muscle movement limitation and painful orbital swelling if the patient has a history of orbital wall fracture repair, even if a bioresorbable implant was used. Prompt imaging evaluation should be emphasized for early diagnosis and proper treatment. J Korean Ophthalmol Soc 2016;57(7):1165-1169
