근시안에서 각막굴절교정 렌즈 처방 후 근시교정량, 안축장길이 및 고위수차와의 상관관계 비교

국문 초록

목적: 근시안에서 각막굴절교정 렌즈(orthokeratology lens) 처방 후 근시교정량 및 안축장 길이 변화를 고위수차 변화와 비교 분석하고자 하였다. 대상과 방법: 각막굴절교정 렌즈를 처방 후 12개월 이상 경과관찰이 가능하였던 환자 18명 36안을 대상으로 처방 전과 처방 후 1개월, 6개월 및 12개월째 시력, 구면렌즈 대응치, 안축장 길이 및 고위수차를 측정하였으며, 이들의 상관관계를 분석하였다. 결과: 각막굴절교정 렌즈 착용 전 전체 고위수차, 구면수차 및 수직 및 수평 코마수차는 각각 0.85 ± 1.47, -0.22 ± 0.40, 0.40 ± 0.47 및 0.22 ± 0.31에서 착용 1개월째 1.11 ± 0.72, 0.10 ± 0.38, 0.79 ± 0.63 및 0.66 ± 1.29로 의미있는 증가소견을 보였으나 (p＜0.05), root mean square (RMS) 총합 및 트레포일수차에서는 의미있는 증가소견을 보이지 않았고(p＞0.05), 이러한 소견은 12개월까지 유지되었다. 또한 전체 및 각막수차 모두 구면수차에서 굴절교정값과 양의 상관관계를 보였으며(각각 p=0.001 및 p=0.028), 안축장 길이와는 음의 상관관계를 보였다(각각 p=0.036 및 p=0.079). 렌즈 착용 전 나안시력 및 구면렌즈 대응치는 각각 0.52 ± 0.08 및 -2.41 ± 0.36에서 착용 1개월째 각각 0.07 ± 0.07 및 -0.84 ± 0.32로 유의한 개선을 보였으며, 12개월까지 유지되었다(각각 p=0.002 및 p=0.001). 안축장 길이는 렌즈착용 전 24.31 ± 0.53 mm에서 렌즈 착용 후 12개월째 24.91 ± 0.60 mm로 지속적으로 증가소견을 보였으나, 통계학적으로 유의하지는 않았다(p=0.721). 결론: 근시안에서 각막굴절교정 렌즈를 착용할 경우 근시의 개선과 동시에 근시량에 비례하여 고위수차가 증가될 수 있으므로 렌즈 처방 시 이를 고려하여야 할 것이며, 근시 억제 기전과 관련하여 고위수차와 안축장 길이와의 관련성에 관해 추후 연구가 뒤따라야 할 것이다. <대한안과학회지 2016;57(7):1050-1055>

영문 초록

Purpose: To report the correlation among the amount of myopic correction, axial length elongation, and higher order aberrations after treatment with orthokeratology lenses in myopic patients. Methods: Eighteen patients (36 eyes) treated with orthokeratology lenses for more than 12 months were recruited for this study. Visual acuity, spherical equivalent, axial length, and higher order aberrations were measured at baseline and 1, 6, and 12 months after wearing lenses. Correlations among them were analyzed. Results: Total higher order, spherical, vertical and horizontal coma aberrations were significantly increased from 0.85 ± 1.47, -0.22 ± 0.40, 0.40 ± 0.47, 0.22 ± 0.31 to 1.11 ± 0.72, 0.10 ± 0.38, 0.79 ± 0.63, 0.66 ± 1.29 after 1 month repectively (p < 0.05), but root mean square (RMS) total and trefoil aberrations were not (p > 0.05), remaining up to 12 months.Total and corneal spherical aberrations were positively correlated to the amount of myopic correction (p = 0.001 and p = 0.028, repectively) and negatively to the amount of axial length elongation (p = 0.036 and p = 0.079, repectively). Uncorrected visual acuity and spherical equivalent were significantly improved from 0.52 ± 0.08 and -2.41 ± 0.36 to 0.07 ± 0.07 and -0.84 ± 0.32 after 1 month respectively. This improvement was maintained up to 12 months (p = 0.002 and p = 0.001, respectively). Axial length was continuously increased from 24.31 ± 0.53 mm to 24.91 ± 0.60 mm after 12 months, but showed no significant changes (p = 0.721). Conclusions: Although orthokeratology lenses were effective for the correction of myopia, they increased higher order aberrations depending on the amount of myopic correction. Physicians should consider higher order aberrations. Further studies regarding the relationship between axial length elongation and higher order aberrations to retard myopic progression should be conducted. J Korean Ophthalmol Soc 2016;57(7):1050-1055
