유피낭종으로 오인된 안와 내 기생충낭 1예

국문 초록

목적: 내측 안와의 유피낭종으로 오인한 기생충 낭종 육아종 1예를 경험하여 이를 보고하고자 한다. 증례요약: 31세 남자 환자가 2년 전부터 서서히 크기가 증가하는 우안 내안각 안쪽 안와의 종괴를 주소로 내원하였다. 환자는 18년 전 동일 부위에서 안와 종양 제거술을 시행한 기왕력이 있었으나, 당시 조직검사는 시행하지 않았다고 하였다. 수술 전 검사한 안와전산화 단층 촬영에서 5.0 × 1.4 × 1.8 cm3 크기의 경계가 명확한 단방성 낭종이 관찰되었고 자기공명영상 T1에서 조영증강되어 일차적으로 유피낭종을 의심하였다. 앞쪽 안와 절개술을 통한 종양 절제술을 시행하였다. 수술 중 낭종이 천공되어 낭 내액을 주사기로 완전히 흡인하고 항생제를 섞은 생리식염수로 안와 내를 철저히 세척하였다. 병리조직 검사에서 분쇄된 기생충 성충이 발견되었고 만성 육아종으로 진단되었다. 결론: 안와 낭종에서 드물게 기생충낭이 진단되기도 하며 이 경우 전신 및 뇌 침범 가능성이 있으므로 주의해야 한다. 이때 유피낭종, 기생충낭 또는 다른 안와종양의 감별이 어려울 수 있어 조직병리 검사가 필수적이다. <대한안과학회지 2016;57(7):1154-1158>

영문 초록

Purpose: To introduce a case of a cyst containing a parasite misdiagnosed as a dermoid cyst, which is to the best of our knowledge, the first report in Korea of a parasite in a cyst located at the medial side of the orbit. Case summary: A 31-year-old male visited the hospital with a 2-year history of a slowly growing mass at the medial side of the right orbit. The patient had a history of mass excision in the same location 18 years previously, however, biopsy was not performed at that time. Orbital computed tomography and magnetic resonance imaging revealed a 5.0 × 1.4 × 1.8 cm3 well-defined T1 high signal intensity unilocular cyst, thus our first impression was a dermoid cyst. The cyst was surgically removed with anterior orbitotomy. The cyst ruptured during the operation, and thus complete aspiration of the cystic fluid and in situ irrigation with antibiotics were performed. Histopathological examination revealed a fragmented adult parasite worm with chronic granulomatous change. Conclusions: A differential diagnosis for orbital cyst based on clinical and radiological results is difficult. Thus, histopathological confirmation is required. A cyst containing a parasite located in the orbit has rarely been reported. A full examination of all infected patients must be conducted for parasite infection.
