속발 내사시에서 수술량을 내사시각의 1/4만큼 시행한 한눈 외직근 전진 수술의 효과

국문 초록

목적: 간헐 외사시로 두눈 외직근 후전 수술 시행 후에 발생한 25 Prism diopters (PD) 이내의 속발 내사시 환자에서 수술량을 내사시각의 1/4만큼 시행한 한눈 외직근 전진 수술의 효과에 대하여 알아보고자 한다. 대상과 방법: 2011년부터 2014년까지 두눈 외직근 후전 수술 후 발생한 속발 내사시로 한눈 외직근 전진 수술을 받고 최소 6개월 이상 경과 관찰이 가능하였던 11명의 환자를 대상으로 의무기록을 후향적으로 분석하여 속발 내사시 수술 전후의 사시각 변화, 수술성공률, 직근 이동에 대한 사시각의 교정 정도를 조사하였다. 결과: 속발 내사시 수술 전 사시각은 원거리 주시시 -19.6 ± 4.7PD, 근거리 주시시 -16.5 ± 7.4PD였다. 한눈 외직근 전진 수술의 수술량은 수술 직전 내사시각의 1/4만큼, 평균 4.8 ± 1.1 mm가 시행되었다. 10명(91%)은 수술 후 정위 및 8PD 이내의 외편위로 회복 되었다. 나머지 1명은 3개월째 내사시가 재발하여 교대 가림 치료 및 프리즘 안경 치료를 병행하여 12개월 후 정위로 회복되었다. 직근 이동에 대한 사시각의 교정량은 수술 후 1일째는 3.3 ± 0.7 PD/mm, 수술 후 1주일째 3.7 ± 0.6 PD/mm, 수술 후 6개월째는 3.8 ± 0.7 PD/mm의 결과를 보였다. 결론: 두눈 외직근 후전 수술 후 발생한 속발 내사시 환자에서, 수술량을 내사시각의 1/4만큼 시행한 한눈 외직근 전진 수술은 모든 경우에서 성공적인 수술결과를 얻었으며, 외직근 이동에 대한 사시각의 교정 정도가 크므로 일차 수술에 비해 수술량을 줄이는 것이바람직할 것으로 생각된다. <대한안과학회지 2016;57(7):1134-1138>

영문 초록

Purpose: To evaluate the efficacy of unilateral lateral rectus muscle advancement surgery based on one-fourth of the angle of consecutive esotropia within 25 prism diopters (PD) which occurred after bilateral lateral rectus muscle recession for intermittent exotropia. Methods: Medical records of 11 patients who underwent unilateral lateral rectus muscle advancement for consecutive esotropia from 2011 to 2014 and who were observed for at least 6 months after surgery were retrospectively reviewed. The change in the angle of deviation before and after consecutive esotropia surgery, success rate, and surgical effect were evaluated. Results: Preoperative esodeviation was -19.6 ± 4.7 PD at distance and -16.5 ± 7.4 PD at near. Unilateral lateral rectus muscle advancement surgeries were performed based on one-fourth of the angle of consecutive esotropia and the mean surgical amount was 4.8 ± 1.1 mm. Of the total 11 patients, 10 patients (91%) recovered to orthotropia or exodeviation within 8 PD. One patient had a recurrence of esotropia at postoperative 3 months, but the patient recovered to orthotropia at postoperative 12 months with alternative patch treatment and a prism glass prescription. The surgical effect of unilateral lateral rectus muscle advancement was 3.3 ± 0.7 PD/mm at postoperative 1 day, 3.7 ± 0.6 PD/mm at postoperative 1 week, and 3.8 ± 0.7 PD/mm at postoperative 6 months. Conclusions: Unilateral lateral rectus muscle advancement surgery based on one-fourth of the angle of consecutive esotropia within 25 PD was successful in all 11 cases. The surgical effect was significantly greater in unilateral lateral rectus muscle advancement than in primary lateral rectus muscle recession. Thus, reduction in the amount of surgery should be considered carefully in unilateral lateral rectus muscle advancement for consecutive esotropia. J Korean Ophthalmol Soc 2016;57(7):1134-1138
