정상 노년층에 대한 인지-언어적 중재 프로그램

국문 초록

고령화 추세로 인해 노인의 인지-언어 능력을 보존하기 위한 다양한 중재들이 시도되고 있다. 그럼에도 불구하고, 정상 노년층을 대상으로 한 인지-언어적 중재의 효과를 체계적으로 살펴본 메타분석 연구는 매우 드문 편이다. 본 연구에서는 정상 노년층에게 적용한 인지-언어적 중재에 관한 국내외 문헌들을 체계적으로 살펴보고, 메타분석을 통해 그 효과성을 검증하고자 하였다. 이를 위해 2000년부터 2015년 사이에 게재된 논문 중 1차적으로 총 679개의 논문을 검토한 후 최종적으로 30개의 논문을 분석하였다. 질적 분석 결과, 중재 프로그램의 하위 영역은 전반적 인지능력, 기억력, 인지 처리속도, 언어능력 등의 순으로 많았다. 중재 결과의 하위 영역은 기억력, 집행기능, 언어능력, 인지 처리속도 등의 순이었다. 메타분석 결과, 인지-언어적 중재의 평균 효과크기는 ‘중간’ 정도로 분석되었다. 중재 프로그램의 하위 영역별로는 기억력과 기능적 인지능력, 그리고 중재 결과의 하위 영역에서는 기억력과 문제해결력이 ‘큰’ 정도로 유의한 효과크기를 보였다. 본 연구를 통해 정상 노년층 대상의 인지-언어적 중재에 대한 증거 기반적 자료를 제공할 수 있었다. 이는 노화에 따른 인지-언어 능력의 저하를 완화시키고, 신경학적 질환의 발병을 감소시키는 데 기여할 임상적 근거를 제시할 수 있다는 데 의의가 있다.

영문 초록

There are increasingly cognitive-linguistic interventions for the elderly according to aging. However, the efficacy of cognitive-linguistic intervention for normal aging has rarely been examined by systematic literature review or meta-analysis. The purpose of this study was to evaluate the efficacy of cognitive-linguistic intervention systematically using a meta-analysis. Thirty studies published since 2000, meeting the inclusion criteria, were entered into the analysis. There were five main findings. Firstly, global cognition held a large portion of the domains in the intervention program, followed by memory, cognitive processing speed, and language, etc. Secondly, most results of treatment included domains like memory, executive function, language, and cognitive processing speed, etc. Thirdly, an average effect size of cognitive-linguistic intervention was moderate. Fourthly, memory and functional cognition in the intervention program were very effective and signigicant. Lastly, memory and problem solving had large and significant effect sizes among domains in the results of treatment. Current study provides the evidence-based information to support cognitive-linguistic rehabilitation of the elderly. This results also can contribute to prevent the cognitive-linguistic decline by aging, and to reduce the neurological diseases.
