장기요양보험 재가급여 여성노인의 요실금 관련요인

국문 초록

본 연구는 장기요양보험 재가급여 여성노인의 요실금 유병률과 요실금 관련요인을 파악하기 위한 서술적 상관관계연구로서, 서울과 경기 10개 지역 방문간호서비스를 이용하는 장기요양보험 재가급여 여성노인 192명을 대상으로 개별면접조사를 실시하였다. Hendrickson(1981)의 도구를 이영숙(1994)이 수정ㆍ보완한 도구로 요실금 정도와 유형을, 표준화한 한국판 단축형 노인우울척도(Geriatric Depression Scale Short Form Korea Version, GDSSF-K)로 우울수준을, 한국판 간이정신상태 검사도구(K-MMSE)로 인지기능정도를, 한국어판 일상생활수행능력 척도로 일상생활수행의존도를 측정하였다. 대상자는 연령이 높을수록, 요양 3등급 대상자보다 2등급 대상자가, 재가서비스 이용기간이 길어질수록, 우울이 심할수록, 인지기능이 낮을수록, 일상생활수행의존도가 높을수록, 대상자가 진단받은 노인성 질환갯수가 많을수록, 손자녀하고만 동거하는 경우, 치매가 있는 노인이 요실금 정도가 심하며, 대상자의 요실금 관련 요인은 우울, 인지기능 및 노인성 질환갯수인 것으로 나타났다. 따라서 장기요양보험 재가급여 여성노인들의 삶의 질을 높이기 위해서 요실금 증상을 단순한 배설물 관리보다는 이들의 잔존기능을 최대한 유지시킬 수 있는 체계적인 관리가 필요할 것으로 본다.

영문 초록

Purpose: This study was performed to investigate the prevalence of urinary incontinence and its associated factors among community dwelling women beneficiaries of long-term care insurance. Methods: The data was collected from 192 elderly women dwelling in community who were beneficiaries of LTC insurance in Seoul and Gyeonggi province from March 1 to April 15. Descriptive statistics, one-way ANOVA, independent t-tests, and stepwise multiple regression analyses were performed using the SPSS Windows 19.0 program. Results: The prevalence of UI among respondents was 79.7%, and showed moderate severity of UI with mixed UI type. The severity of UI in the group with of was significantly severe different according to age(F=4.48, p=.013), grading of LTC(F=14.05, p<.001), duration of home service usage(F=4.92. p=.001), household type(F=2.63, p=.037), ADL(t=3.70, p<.001), depression(F=12.36, p<.001), cognitive function(F=15.06, p<.001), number of diseases(F=6.55, p=.002) and dementia(t=-4.34, p<.001). The adjustment by multiple regression for each statistically different variable in this study showed that depression, cognitive function and the number of geriatric diseases were significant predictors explaining 22% of the variance in UI severity. Conclusion: Proactive nursing interventions of UI should be developed and provided with the consideration of their depression, cognitive function and geriatric diseases.
