치매 노인 여성의 체험연구

국문 초록

본 연구의 목적은 치매 노인 여성의 생생한 삶의 경험과 그 경험에 대하여 어떤 의미를 부여하는지, 치매 노인 여성 경험의 본질은 무엇인지를 이해함으로써, 치매를 경험하는 노인을 깊이 이해하기 위함이다. 본 연구에서는 간호학, 사회복지학, 심리학의 다학제적 접근과 질적연구 전략을 통해 치매노인의 인식 속에 숨겨진 영역들을 찾고자 Max van Manen(1984)의 해석학적 현상학적 분석방법을 사용하였다. 자료수집은 K시의 치매노인 주간보호소에서 65세-80대의 초기 치매노인여성 6명에 대하여 2-3회의 심층면담을 통하여 이루어졌다. 분석결과 치매노인의 경험은 '내치고 싶은 뜻밖의 불청객, '놓치고 싶지 않은 기억의 끝자락', '끊임없이 속으로만 타고 드는 겻불', '흑백사진 속의 생가(生家)', '옥신각신하는 삶과 죽음의 싸움판, '변치 않는 어미로서의 마음'의 6개 핵심 주제로 나뉘었다. 치매 노인 여성은 과거의 기억과 경험을 통해 심리적 위안을 받고, 습관의 유지를 통해 기억 상실상태에 대처하려고 하는 것을 발견하였다. 특히 여성 노인들이 치매를 겪는 가운데에도 지속적이고 변함없는 '모성'을 발휘하고 있었다는 사실의 발견은, 그 동안 보호 대상자로서만 인식되어 온 치매 노인 여성에 대하여 매우 새로운 시각을 제공하고 있다. 이러한 연구결과는 전체적이고 독특한 존재로서 치매노인에 대한 지식을 제공한다. 또한 한국의 가족과 전문가들이 치매노인과 관계를 유지하고, 지속적인 보호제공을 추진할 수 있도록 도울 것이다.

영문 초록

This study illustrates the essential understanding of the psycho-sociological experience of elderly women with dementia. The aim of this study is to deepen family & professional's understanding of the importance of each patient's suffering and future hopes. In this study, an interdisciplinary approach to individual development is combined with the qualitative research strategy of interviewing in order to examine hidden dimensions in the dementia's perception. The study employs a Max Van Manen's phenomenological method. The data analysis follows the procedure of qualitative content analysis. 6 elderly women in their 65-80s were individually observed and interviewed twice at Day Care Center of K County. The experience can be divided into six themes ; Life with death, Edge of memory, Silent tears, Touch in the dark room, Unexpected caller, and Sick mother's affection. From the forgoing, we found that the elderly women with dementia continue to possess some degree of insight and try to cope with what is happening. They concern that they are something wrong with themselves. They feel fear, grief, loss, embarrassment, insecurity, isolation and coming death. This result conveys to us knowledge about the dementia as unique and whole persons. Thus, this awareness stimulates family and professional to maintain the relationship with the care receivers and to initiate an ongoing caring process in Corea.
