노인의 가정 간호요구에 관한 연구

국문 초록

본 연구는 영세민과 독거노인의 가정간호 요구도를 포괄적으로 조사하고 지역사회 의료전달 체계와 가정간호 사업의 전략을 세우는데 기초자료를 제공하고자 한다. 연구대상은 65세 이상의 노인 145명이었으며 UTMB의 Home Health Agency에서 사용한 가정간호요구 도구를 수정 보완하여 가정간호 요구를 측정하였다. 연구결과를 보면 노인들은 93.1%가 질병이 있는 것으로 나타났고 가정간호요구는 영적요구, 심리적 요구, 가정환경관련요구, 신체적 요구, 의사소통 및 건강지식에 관한 요구, 일상생활 활동에 관한 요구의 순으로 나타났다.

영문 초록

The purpose of this study was to explore home health care needs of welfare and/or solitude elderly and to provide the basic data to develop strategies of home health care delivery to the frail elderly residing in community. This is descriptive study and the data were collected in the community. 145 elderly of 65-year-old and more were included in the study and the needs for home health care were questioned in terms of ADL, home envirornment, physical ㆍpsychological ㆍspiritual needs, and communication & health knowledge related needs with modified UTMB. The data were analyzed with descriptive statistics, t-test, ANOVA, and Pearson correlation by means of SPSS program. The results were as follows: 135 subjects(93.1%) were reported to have one and more illness and then comes respiratory, cardiovascular, musculoskeletal, and neural disease in that order; 85 subjects(58.6%) have caregivers available and most of them are family members ; Spiritual needs topped the list, followed by psychological, home environmental, physical, communication & health knowledge related, ADL related needs. Followings are the result of ANOVA, examining difference in prevalence of home health care needs among general characteristics of the respondents; 1) There were significant differences in prevalence of need for ADL among education, family members, insurance, available medical service, financial and residential status. 2) There were significant differences in prevalence of need for home environment related needs among marital status, insurance, available medical service and caregivers, and allowance & financial status. 3) There were significant differences in prevalence of need for physical needs among education, insurance, available medical service, and financial status. 4) There were significant differences in prevalence of need for communication and health knowledge among religion, available medical service, medical history, and residential status. 5) There were significant differences in prevalence of need for spiritual needs among religion, available medical service and caregivers, income resources, and allowance. In conclusion, above characteristics of home health care needs of welfare and/or solitude elderly in the community should be considered as basis in delivering home health care in practice.
