국문 초록

본 연구는 Andersen 행동모형을 분석틀로 이용하여 한국노인의 요양시설 및 재가서비스이용의사와 그 영향요인을 분석한 연구이다. 2004년도 전국 노인생활실태 및 복지욕구조사의 자료를 이용하였으며 연구대상은 65세이상 노인 3,019명이었다. 장기요양 서비스이용의사의 예측요인을 분석하기 위하여 Andersen 행동모형에 근거하여 소인성 요인(연령, 성별, 결혼상태, 교육정도), 가능성 요인(가족월수입, 주관적 경제수준, 직업유무, 자녀수, 가족형태, 거주지), 필요요인(주관적 건강상태, ADL, IADL, MMSE, 행동문제, 만성질환수)을 독립변수로 선택하였다. 전체 대상자의 29.7%가 요양시설 이용의사를 나타냈고, 45.7%는 재가서비스 이용의사를 나타냈다. 연령이 낮고 교육수준이 높고, 자녀수가 적은 도시거주자들일수록 요양시설이용의사가 높은 반면, 연령이 낮고 가족수입이 적으며 주관적으로 인식한 경제상태가 어려운 농촌거주자들일수록 재가서비스 이용의사가 높았다. 본 연구결과는 한국을 비롯한 고령화 국가들의 노인장기요양제도의 발전과 개선을 위한 근거자료로 의미가 있을 것으로 사료된다.

영문 초록

Purpose: To estimate future demand for nursing homes and home care services by Koreans, the intention to use nursing homes and home care services was estimated, and the underlying influencing factors were explored. Methods: Data from the Korean National Survey of Long-Term Care Need in the Elderly compiled in 2004 were used, and 3,019 people aged 65 and older for whom complete data were available were sampled. The following influencing variables were selected based on Anderson's behavioral model of health service use: predisposing factors(age, gender, marital status, education), enabling factors(family income, number of children, place of residence), and need factors(self-rated health, ADL, IADL, MMSE, comorbidity). Results: The result shows that, of all the participants, 29.7% intended to use nursing homes, and 45.7% intended to use home care services. Multivariate analysis revealed that younger age, more education, fewer children, and urban residents were significantly associated with the intention to use nursing homes. On the other hand, the intention to use home care services was associated with younger age, lower family income, and rural residents. Conclusion: Our results will contribute to the development or reformation of long-term care systems in countries with ageing populations.
