노년기 인지기능과 우울수준이 일상생활기능장애에 미치는 영향

국문 초록

본 연구의 목적은 노동연구원의 「한국고령화연구패널(Korea Longitudinal Study of Aging: KLoSA)」 1차(2006년)와 2차(2008년) 기본조사 자료를 사용하여 노년기 인지기능과 우울수준이 일상생활기능장애에 미치는 영향을 분석하는 것이다. 연구대상은 1차와 2차 조사에 모두 참여한 65세 이상 노인 총 3,486명으로 남성이 1,455명(41.7%), 여성이 2,031명(58.3%)이다. 연구결과를 살펴보면 첫째, 교차분석결과 노년기 인지기능수준이 낮은 노인이 우울증이 있는 경우는 그렇지 않은 경우에 비해 일상생활기능장애 수준이 높은 것으로 나타났다. 둘째, 위계적 회귀분석결과 우울수준이 높을수록, 인지기능수준이 낮을수록, 일상생활기능장애 수준이 높은 것으로 나타났다. 셋째, 인지기능과 우울은 장애수준에 상호작용효과를 보이고 있는데, 이러한 상호작용효과는 2006년 장애수준뿐만 아니라 2년 후 장애수준에도 유의미한 영향을 미치는 것으로 나타났다. 치매의심 증상을 갖고 있는 노인뿐만 아니라 그 전 단계인 인지기능저하를 경험하는 노인들도 우울수준이 높으면 그렇지 않은 경우에 비해 2년 후 장애수준이 더 악화되는 것으로 나타났다. 본 연구결과를 통하여 인지기능이 우울과 상호작용을 하여 일상생활기능장애에 영향을 미치고 있으며, 따라서 우울수준을 낮춤으로써 인지기능저하로 인한 장애발생을 지연시키는 역할을 할 수 있음을 알 수 있다. 이에 본 연구에서는 노년기 인지기능수준에 따른 우울증상 관리를 위해 체계적인 개입이 필요함을 제시하고 있다.

영문 초록

The purpose of this study is to investigate the effects of cognitive functioning and depressive symptoms on ADL/IADL disability in the Korean old adults. Samples consist of 1,455(42.7%) men and 2,031(58.3%) women aged 65 above who participated in the 1st and 2nd wave of 「the Korea Longitudinal Study of Aging: KLoSA」. As the results, it's first found that differences of cognitive functioning and depressive symptoms are associated with the level of ADL/IADL disability. A decline of cognitive status is associated with more elevated ADL/IADL scores when depressive symptoms become worse. Secondly, major risk factors for ADL/IADL disability include cognitive functioning and depressive symptoms by means of multiple regression, adjusted for covariates. In addition, the interaction of cognitive status and depression on the ADL/IADL disability is statistically significant not only at 1st wave but also at 2nd wave. These findings suggest that reduction of depression might slow down the effect of cognitive decline on ADL/IADL disability. Expecially, it is necessary to do early detection and intervention of depression in the beginning of cognitive status decline before the emergence of dementia in order to maintain the active and independent life of the elderly.
