국문 초록

말기환자는 죽음 앞에서 자신의 존재에 대해 깊은 상념을 지닌 사람이다. 이들은 현대의학으로는 치유가 불가능한 병으로 인해 신체적, 사회 심리적 그리고 영적인 소외를 경험한다. 이러한 경험은 말기환자를 자기상실의 고통과 두려움에 빠뜨린다. 이 같은 상황에 직면한 말기환자들을 위해 할 수 있는 것은 희망을 안겨주는 것이다. 그 희망은 초월에서 찾을 수 있다. 한계와 나약함을 지닌 인간존재를 넘고 시간과 공간을 넘어 구원이라는 영원(永遠)안으로 인도할 때, 인간존재는 새로운 세계에 대한 희망과 꿈을 갖게 될 것이다. 알폰소 데켄은 말기환자에게 희망을 가져다주는 최상의 방법으로 자율성의 존중을 제안한다. 자율성을 잃어버린 환자는 자기존재상실의 위기에 직면하게 된다. 이러한 환자는 스스로 자신의 존재의 필요성을 잃어버려 마치 있으나 마나한 불필요한 존재처럼 자신을 인식하게 되어 우울증적 반응을 보이거나 심지어 자살을 시도하기도 한다. 이처럼 자율성존중이란 인간이 존엄성을 지닌 가치 있는 존재가 되기 위해 가장 중요한 윤리원칙이다. 말기환자들의 자율성을 존중하기 위한 방법으로는 우선적으로 환자를 존엄한 존재로 인식하고 존중하는 자세 곧 전인적인 이해가 필요하다. 환자에 대한 전인적 이해는 영적 돌봄(spiritual care)을 통해 이루어진다. 영적 돌봄은 한 사람의 신체적인 면과 사회심리적인 면 그리고 정신적인 면 모두를 포함한다. 또한 지나온 과거(history)와 앞으로 다가올 미래까지도 포괄한다. 그리고 한 개인이 가지고 있는 삶의 무게, 가치관, 세상과의 관계 그리고 그 안에서 얻게 된 아픔과 상처 그리고 치유까지도 영적 돌봄의 영역이다. 그러므로 영적 돌봄은 한 사람의 존엄성을 존중하며 세상에 존재할 가치가 있는 사람으로 인정하는 사건이다. 이 사건은 말기환자에게 스스로 새로운 힘을 갖게 하고 자신의 존재가치를 인정받음으로서 새로운 삶(구원)으로의 여행을 준비할 수 있게 한다. 이러한 영적 돌봄은 영적 돌봄 전문가-성직자-만의 몫이라기보다 환자를 돌보는 모든 의료인과 봉사자들 그리고 가족들의 몫이기도 하다. 그러므로 말기환자의 자율성을 존중하는 일과 영적으로 돌보는 일은 같은 맥락에서 다루어져야 한다.

영문 초록

Terminal patients are the ones who face death and have question on their existence. They experience physical, social, psychological and spiritual isolation because of the disease which modern medicine cannot cure. This experience also leads them to suffering and fear about their self-loss. The kind care to these people is to help them to have hope, which comes from the world transcendental. They can have hope when they see the world which transcends the limits and weaknesses of human conditions and the limits of space and time, and when they see the new reality of eternity which is salvation. Alphonso Decken suggests Respect of Autonomy as the best way to give hope to the terminal patients. The patients who have lost their autonomy face crisis of self-loss. They tend to develop depression or often commit a suicide, feeling themselves unnecessary. That is why this respect of autonomy is one of the basic ethical principles and conditions for dignified human beings. One of the ways to respect autonomy of the terminal patients is to recognize them as dignified human beings, in other words, to understand them as a whole person. This comes from spiritual care. Spiritual care is to care the persons physically, socio-psycologically and spiritually. This includes dealing matters of the past and the future. It also includes dealing with pains and wounds and healing. Thus spiritual care attempts to respect and recognize the terminal patients as dignified, and valuable human beings. This encounter helps them to have courage, and prepares them to continue a journey into eternal life. This spiritual care is not a work only of the clergy, but is a work of those who provide medical treatment, voluntary helpers and the family. Therefore respect of autonomy and spiritual care for the terminal patients need to be dealt in the same context.
