동사의미역강화중재가 실어증 환자의 동사 및 명사 이름대기에 미치는 효과

국문 초록

배경 및 목적: 본 연구는 실어증 환자를 대상으로 동사 의미역을 활용한 동사네트워크강화 중재를 통해 동사이름대기 과제에서 수행력의 차이가 나타나는지 살펴보았다. 방법: 실어증 중증도가 경도에서 중도에 해당하는 좌반구 손상에 기인한 실어증 환자 3명이 본 연구에 참여하였다. 실어증 환자에게 동사 의미역을 활용한 동사강화중재(Verb Network Strengthening Treatment)를 실시하여 중재한 동사의 효과크기와 중재하지 않은 의미 관련 동사로의 일반화 효과를 비교하였으며, 또한 명사이름대기 및 실어증지수의 변화를 사전-사후평가를 통해 비교하였다. 결과: 대상자 3명 모두에서 중재효과를 나타났으며, 특히 대상자 1의 경우 큰 중재 효과크기가 관찰되었다. 또한 중재하지 않은 의미 관련 동사 이름대기 자극에서도 대상자 모두 일반화 효과를 보였으며, 명사이름대기와 실어증지수에서의 사전-사후검사에서도 수행력이 증가한 것으로 나타났다. 논의 및 결론: 본 연구의 결과에 따르면, 동사 의미역을 활용한 네트워크강화중재는 중재한 동사뿐만 아니라 중재하지 않은 의미 관련 동사에까지 일반화를 가져오는 효과적인 중재방법인 것으로 나타났다. 이는 기존의 영어권 선행연구와 일치하는 결과로서, 한국어 동사중재에서도 의미역을 활용하는 것이 실어증 환자들의 동사산출능력 강화에 도움을 줄 수 있을 것으로 기대된다.

영문 초록

Objectives: The Verb Network Strengthening Treatment (VNeST) is a semantic treatment to improve lexical retrieval of content words in sentence context by promoting systematic retrieval of verbs and their thematic roles. The purpose of this study was to examine the effects of VNeST on Korean individuals with mild to moderate aphasia. Methods: Three Korean individuals with aphasia due to a stroke in the left hemisphere participated in the study. The treatment followed the steps of 1) naming of the target verbs, 2) generation of three agents or themes for target verb, 3) generation of corresponding agents or theme, 4) answering to wh-questions about agent-theme pairs, 5) semantic judgment of each sentence, and 6) naming of the target verbs. Multiple-baseline design across participants was employed. Effect sizes of the treatment were also obtained. Results: Results demonstrated medium to large effect sizes on trained verbs in all participants. They also presented generalization effects on untrained verbs. In addition, participants exhibited better performance on noun naming tasks, and aphasia quotient increased in the post-treatment phases compared to the pre-treatment phase. Conclusion: The current study suggested that the VNeST was effective for individuals with aphasia. The results were consistent with previous findings from English speaking individuals with aphasia. Further studies are required to validate the treatment efficacy by including more participants with various severity and types of aphasia.
