뇌졸중 환자의 낙상 경험에 따른 균형, 운동성, 균형자신감의 비교

국문 초록

본 연구는 낙상 경험에 따른 뇌졸중 환자의 균형, 운동성, 균형자신감을 비교하는데 목적이 있다. 연구의 대상자는 독립보행이 가능한 46명의 편마비 환자이며, 낙상경험을 기준으로 낙상군과 비낙상군으로 분류하여 균형, 운동성, 균형자신감을 비교하였다. 균형능력은 버그균형척도(Berg balance scale, BBS), 3가지 상태(TUG-alone, TUG-cognitive, TUG-manual)에서의 일어나 걸어가기 검사(Timed-up-and-go, TUG)를 실시하였고, 낙상관련 운동성은 Tinetti 보행-균형검사(Tinetti Gait & Balance Scale), 그리고 균형자신감은 활동-특정 균형자신감 척도(Activities- specific balance condfidence scale, ABC)를 사용하여 평가하였다. 연구결과 전체 대상자의 TUG의 상태에 따라 TUG의 평가 결과의 차이가 있었으며, 낙상 경험 유무에 따라 BBS(p <.05), TUG-alone(p<.01), TUG-cognitive(p <.05), TUG-manual(p <.05)에서 유의한 차이를 보였다. 또한 균형, 운동성, 균형자신감 간의 상관관계에서는 BBS, TUG-alone, TUG-cognitive, TUG-manual, Tinetti-Gait, Tinetti-Balance, Tinetti-total, 그리고 ABC 사이에 상호 간 유의한 상관관계를 보였다(p <.01). 본 연구의 결과를 통하여 낙상경험은 균형 능력과 관계가 있음을 확인하였다. 앞으로 균형능력 뿐만 아니라 뇌졸중 환자의 낙상을 일으키는 여러 가지 요인에 대한 깊이 있는 연구가 필요하며, 낙상을 감소시키기 위한 균형훈련 프로그램의 개발이 적극적으로 필요하다고 사료된다.

영문 초록

The purpose of this study was to comparison of balance, mobility and balance-confidence in stroke patients with fallers and nonfallers. The subjects used in this study were 46 patients with hemiparesis being able to walk independently. Among them, fallers were idenfified, and they answered the question in the number of falls. On the basis of their falls, patients were divided into two groups; one group was for fallers and the other was for non-fallers. The balance function assessments were used to measure Berg balance scale(BBS), Timed-up-andgo, (TUG) under 3 condition(TUG, TUG with a subtraction task(TUG-cognitive), and TUG while carrying a full cup of water(TUG-manual) were used. Mobility assessments were used to measure the Tinetti Blance(TB), Tinetti Gait(TG) and Tinetti Total(TT). And balnce confidence was measured the Activities- specific balance condfidence scale(ABC). The two group were compared using independent t-test and the data were analyzed using Pearson product conrrelation. As a result, For TUG under 3 condition, The TUG showed difference in TUG, TUG-C and TUG-M. The fallers group showed significantly poorer score in BBS, TUG-A, TUG-C and TUG-M. In correlation analysis, all variables was significantly correlated with each other(p <.01). These result suggest that falls is related to balance function in stroke patients.
