뇌졸중 환자의 흡인지표 개발을 위한 기초 연구

국문 초록

본 연구의 목적은 뇌졸중 환자들의 흡인과 관련된 지표를 개발하기 위하여 Stage Transition Duration과 Pharyngeal Transit Time을 측정하였다. STD와 PTT의 정량화를 위하여 VFSS의 결과로 얻어진 영상을 GraphClick 3.0을 이용하여 측정, 분석하였다. 연구에 참여한 대상은 총 30명으로 뇌간에 병변이 있는 뇌졸중 환자 중 흡인을 보인 10명(aspirator), 흡인을 보이지 않은 10명(non aspirator), 삼킴장애 및 기타 신경학적 진단을 받지 않은 정상군 10명이 포함되었다. 각 집단의 평균을 비교한 결과, STD와 PTT 모두 세 군 간에 차이를 보이지 않았다. 그러나 사후검정에서는 STD에서 흡인자와 비흡인자 간에 차이가 있는 것으로 나타나 향후 유의미한 차이를 보일 가능성을 시사하였다. 각 시간적 지표의 예비연구를 통해 관련된 해부학적 구조의 유기적 관련성에 따라 달라짐을 유추하였다.

영문 초록

The purposes of the current study were to investigate STD and PTT as a predictive factor of dysphagia after stroke. Three groups of subjects were examined: 1) 10 brainstem stroke patients who aspirated (aspirator), 2) 10brainstem stroke patients who did not aspirate (nonaspirator), 3) 10 normal subjects. To examine the participant’s swallowing function, an instrumental study was conducted with the videofluoroscopy. And Graphclick 3.0 were used to quantitatively measure the stage transit duration (STD), and the pharyngeal transit time (PTT) shown from the video data. As a result, there were no statistically significant differences among three groups. However, aspirator tended to have a longer STD than nonaspirator. These study suggest that further research should be directed at looking up to a greater number of subjects in order to increase the power of the result, and should be investigate the various structure and function to explain the organic relevance of them.
