구개열 영유아를 위한 부모 중심 조기언어중재가 말-언어발달에 미치는 효과

국문 초록

배경 및 목적: 구개열 영유아의 말-언어발달 패턴을 살펴본 선행연구들은 구개열 영유아들은 언어이전기 단계에서부터 느리거나 다른 발달 양상을 보이고, 초기 말-언어발달 단계에서도 이러한 특징은 연속적으로 나타난다고 하였다. 이러한 연구결과는 구개열 영유아를 위해 말-언어발달상의 지연이 악화되는 것을 예방하고 정상적인 발달을 촉진하는 조기 언어중재가 필요함을 시사하고 있다. 본 연구는 구개열 영유아의 말-언어발달 특성과 부모 중심 조기언어중재와 관련된 선행 연구를 바탕으로 구개열 영유아를 위한 부모 중심 조기 언어중재프로그램을 개발, 실시하여 어머니의 상호작용 방식과 영유아의 말-언어발달에 미치는 영향을 살펴봄으로써 프로그램의 효과를 검증하고자 하였다. 방법: 생후 12∼30개월 사이의 구개열 영유아와 어머니 11쌍이 연구에 참여하였다. 정상적인 말-언어발달 패턴 및 구개열 영유아의 말-언어 특징에 대한 정보와 언어자극 기법과 상호작용 핵심전략 훈련 등의 내용을 포함한 부모교육을 실시한 후 3개월 간 지속적인 모니터링과 상담을 통해 가정 내에서 부모중심 언어중재를 실시하게 하였다. 부모교육 프로그램 전과 후에 실시된 말-언어평가를 통해 어머니의 상호작용 방식과 아동의 말-언어발달에 어떠한 차이가 있는지 살펴보았다. 결과: 부모교육프로그램 이후 아동과의 상호작용에서 어머니가 사용하는 서로 다른 낱말수가 유의미하게 감소하였고, 아동의 의사소통 의도에 대한 어머니의 반응하기가 통계적으로 유의하게 증가하였다. 반응하기 하위 영역 중에서 언어발달에 긍정적인 영향을 끼치는 언어적 연결, 수정 피드백, 확장 및 확대 사용이 유의하게 증가했다. 어머니의 상호작용 핵심전략 사용여부에 대해 핵심전략 3개의 항목과 전체 평점에서 평가자들에게 통계적으로 유의하게 높은 점수를 받아 부모교육 이후 어머니들이 아동과의 상호작용에서 아동의 말-언어발달에 긍정적인 기술이나 전략을 사용하는 것으로 나타났다. 프로그램 이후 아동의 말-언어발달 수준은 SELSI 등가연령, MBCDI-K 표현어휘수, 의사소통 방법에서 명료한 구어 사용의 빈도, 자음목록수, 파열음과 유음의 산출, 음절구조다양도 항목에서 통계적으로 유의한 증가를 보였다. 논의 및 결론: 본 연구의 결과는 부모 교육을 통한 가정 내 조기 언어중재가 부모의 상호작용 방식상의 긍정적인 변화를 일으키고 결과적으로 구개열 영유아들의 표현 언어와 말 산출 능력이 향상되는데 영향을 끼쳤음을 제안하고 있다.

영문 초록

Background & Objectives: Young children with cleft palate have a high risk of speech-language impairment due to innate anomalies of the oral mechanism, which indicate the need for early intervention for speech impairment. The purpose of the present study was to explore the effectiveness of a parent-implemented early intervention on parents’ interaction styles and children’s speech-language development. Methods: Eleven pairs of mothers and children between 12 and 36 months of age participated in the present study. After the mothers attended parent training that focused on language stimulation skills and communication facilitation strategies, parent-implemented intervention was performed in the children’s home for three months through regular monitoring, coaching and counseling. Parents’ interaction styles and children’s speech-language characteristics were compared pre- and post-intervention tests to investigate the effectiveness of a parent-implemented early intervention. Results: The number of different words in the mothers’ utterances significantly decreased, and their responsiveness to children’s communication acts significantly increased. Mothers showed more frequent uses of linguistic mapping, corrective feedback, linguistic extension and expansion among subcategories of responsiveness after the intervention. Rating scores on maternal use of communication facilitation skills indicated that mothers more consistently used the learned skills when interacting with their children following the parent training. Among child speech-language measures, language age of SELSI, number of expressive words, percentage of intelligible speech, consonant inventory and number of different syllable structures significantly increased after the intervention. Discussion & Conclusion: The results of the present study support the effectiveness of parent-implemented early intervention on positive changes of parents’ interaction styles and children’s speech-language development.
