절차공정성과 정신장애인의 회복태도와의 관계에서 강요경험의 매개효과

국문 초록

본 연구의 주요 목적은 입원이나 치료과정에서 발생하는 절차공정성의 문제와 회복태도, 강요경험에 대한 정신장애인의 인구학적ㆍE장애특성별 차이를 알아보고, 절차공정성과 회복태도와의 관계에서 강요경험(긍정 및 부정)의 매개효과를 살펴보는 것이다. 대상은 입원 중이거나 지역사회정신보건센터를 이용하는 정신장애인 391명이다. 분석은 각 변수의 집단차이와 상관성을 보기 위해 t-test와 상관관계분석을 실시하였다. 그리고 Baron과 Kenny의 4단계 회귀분석 및 sobel test를 활용하여 매개효과를 분석하였다. 여기서 기존연구들에서 회복에 강력한 예측요인으로 보고한 증상은 통제되었다. 연구결과, 첫째, 절차공정성과 긍정적 강요경험은 지역사회거주 집단에서, 부정적 강요경험은 입원집단에서 높았다. 경제수준과 긍정적 강요경험은 정적 상관을, 교육수준과 부정적 강요경험은 부적 상관을 보였다. 증상은 절차공정성과 회복태도와는 정적 상관을, 긍정 및 부정적 강요경험과는 부적 상관을 보였다. 둘째, 절차공정성과 회복태도와의 관계에서 긍정적 강요경험은 부분매개효과를 보인 반면, 부정적 강요경험의 매개효과는 검증되지 않았다. 이러한 결과를 바탕으로 정신장애인의 강요경험의 감소와 자율성 및 회복 증진을 위해 치료전 과정을 걸친 절차공정성 원칙의 강화, 정신보건전문가 뿐 아니라 타 전문가들의 활용, 사전계획서 방식의 활용 등을 제시하였다.

영문 초록

The main objectives of this study are to estimate differences of procedural justice, recovery attitude and coercion experience between socio-demographic and disorder characteristics, and to identify the mediating effect of coercion experience on the relationship between procedural justice and recovery attitudes of the persons with mental illness. Subjects consisted of 391 persons with mental illness who were hospitalized and resided in the community. We used t-tests and regression analysis through the four-step procedures by Baron and Kenny and sobel test to identify the mediating effects of coercion experience. The major findings of this study were: First, community group is higher positive coercion experience and procedural justice than hospital group. Hospital group is higher negative coercion experience than community group. There is a significant correlation the economic level and the positive coercion experience and a significant correlation the educational level and the negative coercion experience. The mental symptoms was significantly associated with procedural justice, coercion experience and recovery attitudes. Second, positive coercion experience worked as partial mediators on the relationship between procedural justices and recovery attitudes, whereas the negative coercion experience didn't work as mediators. Based on these finding, an enhancing of procedural justice principle, a continuing training and education in mental health professionals, utilizations of other professionals and the form of advance directives were discussed.
