음성장애 진단 및 평가의 최근 연구 동향

국문 초록

배경 및 목적: 음성장애의 진단 및 평가 기법은 과학 및 기기의 발달과 더불어 나날이 변화하고 있다. 이러한 흐름에 발맞추어 최근의 연구 동향을 파악함으로써 임상과 교육 및 연구에 활용 할 수 있는 자료를 제시하고자 하였다. 방법: 2005년부터 2010년까지 『4대한음성언어의학회지』, Journal of Voice 등 국내외에서 발표된 음성장애 진단 및 평가 논문 중 73편의 논문을 선정하였다. 이들을 청지각적 평가, 기계적 평가, 자기보고식 평가, 둘 이상의 평가 방법에 대한 비교, 기타로 분류하여 분석하였다. 결과: 청지각적 평가에 대한 논문 9편, 기계적 평가에 대한 논문 37편, 자기보고식 평가에 대한 논문 13편, 청지각적 평가와 기계적 평가를 비교한 논문6편, 청지각적 평가와 자기보고식 평가를 비교한 논문 2편, 기계적 평가와 자기보고식 평가를 비교한 논문 5편, 기타 논문 2편이 분석되었다. 청지각적 평가에서는 평가의 신뢰도를 높이고자 하는 시도가 지속되고 있으며 기계적 평가에서는 비선형 동적 분석이나 Phonation Threshold Pressure (PTP)나 Phonation Threshold Flow (PTF)에 대한 유용성이 새롭게 제시되었다. 자기보고식 평가에 있어 국외에서는 기존 도구를 다양한 집단에 적용할 수 있도록 변형하는 작업이 주로 이루어졌고 국내에서는 국외에서 많이 사용되는 도구의 번안작업 및 그 타당도의 연구가 주로 이루어졌다. 단일한 검사가 갖는 부족함을 보충하기 위해 어떤 조합의 검사가 유용한지에 대한 실험도 지속되고 있다. 논의 및 결론: 이러한 추세에 따라 국내에서도 비선형 동적 분석이나 PTP, PTF에 대한 연구가 적극적으로 이루어져야 하며 최선의 결과를 얻기 위해 다양한 검사 방법의 조합에 대한 연구가 이루어져야 한다. 또한 청지각적 평가의 신뢰로운 결과를 얻기 위해서는 외적 기준을 이용한 청지각 훈련이 이루어져 야 한다.

영문 초록

Background & Objectives: Techniques for diagnosis and evaluation of voice disorders are changing daily with the development of science and instruments. To keep pace with this trend, we suggest the identification of recent research trends and useful data from the literature for clinical, educational and research fields. Methods: From the research articles with the topic of voice disorder diagnosis and evaluation published between 2005 and 2010, 73 articles were selected. These articles were published in both domestic and foreign journals including The Journal of The Korean Society of Logopedics and Phoniatrics, and the Journal of Voice. The articles were divided into 5 categories: perceptual evaluation (9 articles), instrumental evaluation (37 articles), self-reporting evaluation (13 articles), comparison among the 3 types of evaluation (12 articles), and others (2 articles). Results: Various methods to improve the reliability of perceptual evaluation have been continuously attempted, and numerous studies of instrumental evaluation have focused on the usefulness of nonlinear dynamic analysis, Phonation Threshold Pressure (PTP), and Phonation Threshold Flow (PTF). In other countries, the trend of using a self-report evaluation was a modification of existing tools for application in more various groups including singers and alaryngeal voice users. The aim in Korea was to translate those tools into Korean and test their validity. To compensate for the insufficiency of a single tool measurement, various attempts were performed to determine what combinations have the best results. Discussion & Conclusion: According to current research trends, nonlinear dynamic analysis, PTP, and PTF should be studied in order to find the best combination of evaluation tools and to improve the students’ perceptual evaluation skills by using external standards.
