국문 초록
청각장애인은 의사전달이 어려운 ‘말 명료도’ 문제와 청자가 화자의 말을 이상하게 여기는 ‘말 용인도’ 문제를 가지고 있다. 그러나 실제 임상의 말 평가는 말 명료도와 분절적 요소에 국한되어 있다. 이에 본 연구는 청각장애 성인의 발화가 초분절적 요소, 말 명료도, 말 용인도 측면에서 청지각적으로 어떻게 평가되는지 살펴보고, 초분절적 요소가 말 명료도와 말 용인도에 미치는 영향을 알아보았다. 방법: 음도, 음량, 음질, 공명, 억양, 말속도의 상위 6개 영역 항목에 속한 하위 22개 구체 항목으로 초분절적 요소에 대한 평가 항목을 작성하였다. 피험자는 고도 이상 난청 성인 55명이었으며, 언어습득 전과 언어습득 후 난청집단, 고도와 심도 난청집단으로 구분되었다. 피험자는 그림설명 과제와 문단 읽기 과제, 무의미음절과 단어 이름대기 과제를 수행하였다. 청각장애 경력이 3년 이상인 언어치료사 5인이 초분절적 요소와 말 명료도, 말 용인도를 100mm 시각적 아날로그 척도에 평가하였다. 결과: 첫째, 초분절적 영역 항목 중증도는 청력 손실의 시기와 정도에 따른 차이가 통계적으로 유의하였다. 언어습득 전이 언어습득 후 난청집단보다, 심도가 고도 난청집단보다 중증도가 높았다. 둘째, 말 명료도와 말 용인도는 언어습득 전이 언어습득 후 난청집단보다 낮았고, 심도가 고도 난청집단보다 낮았다. 말 용인도는 말 명료도에 비하여 다소 낮게 평가되었으며, 두 변인은 전체적으로 높은 상관관계를 보였다. 그러나 언어습득 후 난청집단에서는 말 명료도에 비해 말 용인도가 다양하게 나타나서, 두 변인 간의 상관관계가 유의하지 않았다. 셋째, 말 요소와 말 명료도, 말 용인도의 상관분석 결과, 분절적 요소는 말 명료도와, 초분절적 요소는 말 용인도와 상관계수가 높았다. 다중회귀분석 결과, 말 명료도는 자음정확도, 모음정확도, 억양 중증도, 말속도 중증도가, 말 용인도는 자음정확도, 억양 중증도, 공명 중증도, 말속도 중증도가 유의하게 영향을 미치는 것으로 나타났다. 즉, 초분절적 요소 역시 말 명료도와 말 용인도에 영향을 미치며, 말 용인도에 더 많은 영향을 주고 있음을 알 수 있었다. 결론: 청각장애 성인의 청지각적 말 평가에서 초분절적 요소, 말 명료도, 말 용인도가 청각장애의 다양한 말 문제를 기술하는 데 있어 유용한 지표임을 확인하였다.
영문 초록
Background & Objectives: Patients with hearing impairments (HI) have problems communicating due to the lack of speech intelligibility (SI) and speech acceptability (SA). However, the speech evaluation in clinics is limited to evaluating SI and segmental factors. Hence, the present study examines how the speech of adults with HI are perceptually evaluated in terms of suprasegmental factors, SI, and SA, and specifically how suprasegmental factors may influence SI and SA. Methods: First, the auditory-perceptual suprasegmental profiles were created by including the upper 6 section items such as pitch, loudness, voice quality, resonance, intonation, and speech rate, and the lower 22 concrete items. Fifty-five subjects were separated into 2 groups based on the onset of HL (prelingual vs. postlingual) or the degree of HL (severe vs. profound). Subjects were asked to participate in picture description tasks, paragraph reading tasks, non-syllabic and wordnaming tasks. Five professional speech and language pathologists used the 100 mm Visual Analog Scale to measure the severity of the suprasegmental section items, SI, and SA. Results: First, there were significant differences in suprasegmental factors depending on the onset and the degree of HL; prelingual patients showed a relatively greater degree than postlingual patients, and the profound group demonstrated greater severity than the severe group. Second, SI and SA were relatively lower in the prelingual and profound group than in the postlingual and severe group. In addition, overall SA rates were lower than SI rates. Although these 2 rates showed high correlations in general, no significant orrelations were observed in the postlingual group. Third, Pearson correlation analysis revealed that segmental factors were more highly correlated with SI, but suprasegmental factors were more highly correlated with SA. Results of multiple regression analysis implied that factors such as the percentage of consonants correct (PCC), percentage of vowels correct, and the severity of intonation and speech rates significantly influenced SI. However, PCC, the severity of intonation, and the resonance and speech rates significantly influenced SA. Discussion & Conclusion: The results of the present study verified that suprasegmental factors, SI, and SA are valuable indices that may be applied to the effective evaluation of various speech problems in adults with HI during an auditory-perceptual evaluation of speech.
