심리부검 연구의 실무적 활용 및 윤리적 고려사항

국문 초록

  심리부검은 자살자에 대해 수집된 포괄적인 후향적 정보를 가지고 자살에 대해 연구하는 방법이다. 심리부검은 자살에 대해 가장 유용한 연구 방법 중 하나이다. 심리부검은 일단 자살자의 가족 구성원, 친척, 친구, 직장동료, 담당 의료인 등 기타 지인들에 대한 구조화된 면접을 통해서 자살자에 대한 모든 활용 가능한 정보를 수집한다. 추가적으로 모든 활용 가능한 의료 기록 및 정신과 치료 기록, 검시결과와 기타 자료 등을 활용한다. 따라서 심리부검은 다양한 피조사자와 기록들로부터 정보를 종합하는 것이다. 또한 심리부검은 사망의 종류를 결정하는데 결정적인 도움을 줄 수 있다. 즉, 심리부검을 통해서 자살 사건이 자연사인지, 사고사인지, 자살인지, 타살인지 결정하는데 도움이 된다.
  초기의 심리부검 연구들은 자살자의 90% 이상은 공통적인 정신장애를 겪으며, 그러한 정신장애의 대부분은 기분장애 및 물질사용장애라는 사실을 밝혀냈다. 또한 그러한 연구들에 따르면 상당한 수의 자살자가 정신장애에 대해 치료를 받지 못했지만, 종종은 정신과 치료 또는 다른 보건 서비스를 받았음에도 불구하고 자살하였다.
  한편 심리부검에 있어서 조사를 설계하고 수행할 때에는 많은 윤리적 고려사항들이 있다. 여기에는 적절한 면접 방법 및 시기의 고려, 비밀과 익명성의 보장, 참여에 대한 동의, 참여자와 연구자의 상호 존중과 신뢰의 구축, 유족에 대한 지원 등이 있다. 특히 피조사자의 반응을 사려 깊게 고려해야만 한다. 피조사자들은 연구가 세심하게 설계되고 수행될 것을 기대할 권리가 있다. 이것은 만일 피조사자들이 자신의 행복 그리고 자신의 권리와 존엄성이 침해되었다고 느낀다면, 면접을 중단할 수 있기 때문에 중요한 문제이다.

영문 초록

  Psychological autopsy is a research method by which comprehensive retrospective information is collected concerning victims of suicide. Psychological autopsy is one of the most valuable research methods on suicide. The method involves collecting all available information on the deceased via structured interviews of family members, relative, friends, attending health care personnel. In addition, information is collected from available health care and psychiatric records, other documents, and forensic examination. So, psychological autopsy synthesizes the information from multiple informants and records. Moreover, psychological autopsy assist to determine the manner of death. It can identify whether suicide is the result of natural causes or accident or suicide or homicide.
  Early studies of psychological autopsy revealed that more than 90% of suicides had been suffered from co-morbid mental disorders. Most of them are mood disorders and substance use disorders. Futhermore, they revealed than the remarkable undertreatment of these mental disorders, often despite contact with psychiatric or other health care services.
  Besides, There are many ethical considerations when designing and carrying out such an investigation, not least the reactions of informants. Ethical considerations are appropriate manner and timing of interview, safeguarding confidentiality and anonymity, informed consent, mutual respect and building of confidence between informants and researchers, support of bereavement. Informants have the right to expect that the research has been sensitively designed and carried out. This is a important point because they can stop the research interview, if they feel their right and dignity have been infringed.
