골성상악견인의 장기관찰과 그 임상적 의미

국문 초록

3급부정교합에 대한 조기치료는 치료후의 잔여성장량에 의해 성패가 가려지게 된다. Tooth borne RME (Rapid Maxillary Expansion)를 고정원으로 하는 상악견인의 장기적관찰들은 상악의 견인량보다는 이후의 하악성장의 양과 방향에 의해 예후가 결정된다는 점을 지적하고 있다. 이에 따라 하악의 성장량을 압도할 장치로서 Bone anchored maxillary protraction (BAMP) 이 등장하게 되었다. 골성지지원이 확보되고 장치의 하루 종일착용이 이루어진다면 BAMP의 사용으로 5군데의 주요골격변화를 관찰할 수 있다. A point, Orbitale와 Nose의 변화는 순수한 상악견인에 의해 발생하며, Gonial angle과 TMJ에서의 변화는 장기간의 장치사용에 의한 Chin cup 효과에 의한다. 이러한 BAMP들은 Orbit와 Nasal bone을 포함하는 상악복합체의 견인량이 기존의 conventional한 facemask보다 클뿐만 아니라, 하악의 변화를 가져오는 Chin cup effect로 인해 상하악의 총체적인 변화가 가능하다. 그러나 하악에서의 변화는 extraoral traction보다는 Intraoral traction에서 현저히 나타나게 된다.

영문 초록

The success of early orthodontic treatment of Class III malocclusion depends on the remaining growth pattern. Several long-term studies on tooth-borne facemasks emphasize that the key factor determining long-term success with a facemask is not solely the response of the maxilla to forward traction, but rather the amount and direction of mandibular growth during and after adolescence. There are five significant skeletal changes resulting from bone-anchored maxillary protraction (BAMP) when bone anchors are utilized and all-day wearing is prescribed. The changes in A point, Orbitale, and Nose primarily arise from genuine maxillary protraction. On the other hand, the modifications in the gonial angle and TMJ are attributed to the chin cup effect, which is intensified by prolonged wearing. BAMP can overwhelm mandibular growth due to the amount of greater maxillary protraction including orbit and nasal bone and chin cup effect for mandibular changes. However, in order to facilitate and sustain mandibular changes, intraoral traction is deemed necessary.
