횡적 부조화 및 전치부 개방교합을 동반한 골격성 II급 부정교합 환자의 치험례

국문 초록

목적: 골격성 II급 경향과 함께 횡적 부조화 및 전치부 개방교합을 가진 장안모 환자에 대해 비수술적인 방법, 즉, 상악궁의 급속 확장과 상악 구치부의 압하, 상·하악 전치열의 후방이동을 시행한 증례를 보고하고자 한다. 증례: 본 27세 여환은 전치부 개방교합을 주소로 본원에 내원하였으며, 그 이외에도 골격성 II급 및 상·하악골 횡적 부조화 경향을 보였다. 먼저 횡적 부조화 해소를 위해 Miniscrew-assisted Rapid Palatal Expansion (MARPE)를 이용한 급속확장을 시행하였으며, 골격성 고정원으로 상악 구치부를 압하하여 하악골의 전상방 회전(autorotation)과 함께 전치부 개방교합을 해소하고자 했고, 마지막으로 상·하악 전치열의 후방이동을 통해 추가적인 수직피개를 확보하고자 했다. 그 결과, 하악골의 성공적인 전상방 회전으로 인해 전후방적, 수직적 골격관계의 개선과 함께 입술폐쇄부전과 근육의 긴장이 해소되었다. 결론: 장안모, 골격성 II급, 전치부 개방교합을 동반한 환자에서 골격성 고정원을 이용하여 상악 구치부의 합입과 함께 하악 구치부의 정출을 방지하면 성공적으로 하악골의 autoration을 유도할 수 있다. 상·하악 치열 전체의 저항중심을 고려하여 후방이동을 시행하면 교합평면의 회전으로 수직피개를 증가시킬 수 있다.

영문 초록

Purpose: We report the case of a skeletal Class II hyperdivergent patient with an anterior open bite and maxillomandibular transverse discrepancy who underwent orthodontic treatment with maxillary expansion, maxillary molar intrusion, and total distalization of the maxillary and mandibular teeth. Case report: A 27-year-old patient came to our clinic for the treatment of an anterior open bite. She also had a Class II malocclusion with a transverse discrepancy. For the transverse discrepancy, rapid maxillary expansion was performed using Miniscrew-assisted Rapid Palatal Expansion (MARPE). Then, we used temporary anchorage devices (TADs) to intrude the maxillary molars, which can cause upward and forward autorotation of the mandible and closure of the anterior open bite. Finally, total distalization of both the maxillary and mandibular teeth was performed to further increase the overbite. Successful autorotation of the mandible resulted in the improvement of the sagittal and vertical skeletal relationships and the alleviation of muscle tension and lip incompetency. Conclusion: In patients with a hyperdivergent profile, Class II malocclusion, and anterior open bite, successful autorotation of the mandible can be induced using TADs to intrude the maxillary molars and prevent extrusion of the mandibular molars. Total distalization of the maxillary and mandibular teeth, with consideration of the center of resistance, can increase overbite by rotating the occlusal plane.
