안면비대칭을 동반한 골격성 III급 부정교합의 상악 분절골 절단술을 동반한 수술횡적교정 치험례

국문 초록

심한 골격성 III급 부정교합과 안면 비대칭을 동반한 환자의 악교정수술 시, 횡적 문제의 해결이 선행되어야 안면 비대칭과 교합 관계를 개선시킬 수 있다. 상악궁의 확장은 수술 또는 비수술적 방법이 모두 가능하지만, 성인 남성에서는 비수술적 확장의 성공률이 매 우 낮은 것으로 보고되어 있다. 본 증례에서는 21세의 남자 환자에서 비수술적 확장 대신, 상악궁을 수술적으로 확장함과 동시에 악교정수술을 시행하여 한 번의 수술만으로 안면 비대칭과 III급 부정교합을 성공적으로 개선한 치료 경과를 보고하고자 한다. 연조직의 저항을 고려하여 상악골의 수술적 확장의 양을 제한하고, 수술 직후 rapid palatal expansion (RPE) 장치를 장착하여 추가적인 확장을 시행하였다. 수술 후 교정치료 동안 botulinum toxin을 편측 교근에만 주사하여 비대칭적으로 발달한 근육의 활성을 조절하고 술후 하악골의 안정적인 위치를 유지하고자 하였다. 총 26개월의 치료기간이 소요되었으며 환자는 횡적, 전후방적으로 양호한 교합을 달성하였고 안면 비대칭이 개선되어 심미적인 안모를 갖게 되었다.

영문 초록

In patients with severe skeletal Class III malocclusion and facial asymmetry, correction of the transverse problem is necessary to improve the occlusal relationship and facial asymmetry by orthognathic surgery. Although maxillary expansion can be achieved by surgical or non-surgical methods, the success rate of non-surgical expansion has been reported to be significantly low in adult males. In this report, Class III malocclusion with facial asymmetry in a 21-year-old male patient was effectively improved with surgical maxillary expansion and simultaneous orthognathic surgery rather than with non-surgical expansion. Surgical expansion of the maxilla is known to be less stable due to resistance from the surrounding soft tissues. To ensure stability and allow additional expansion, a rapid palatal expansion (RPE) device was fabricated before the surgery and delivered immediately after the surgery. Moreover, botulinum toxin was injected only into the masseter muscle on the affected side to control the asymmetrically developed muscles during post-surgical orthodontic treatment. The entire treatment took 26 months, and facial symmetry was successfully corrected with the establishment of good occlusal relationship in the transverse and anteroposterior directions, resulting in an esthetic appearance.
