후천적 신체 장애인의 장애기간에 따른 장애수용도의 변화와 장애 정도의 조절효과

국문 초록

본 연구는 후천적 신체장애인의 장애 기간에 따른 장애수용도의 변화를 파악하고, 이러한 변화에 중증과 경증으로 나누어지는 장애 정도가 유의미한 조절 효과를 보이는지 검증하는 것을 목적으로 한다. 특히, 애도 및 장애수용의 단계 이론에 근거하여, 인구통계학적 특성 및 사회경제적 요인과 주관적 건강 만족도를 통제한 후 중증과 경증 장애인의 장애수용도 변화를 분석하여, 장애인의 정신건강 증진을 위한 실천적 개입 방법의 효과성을 높이고자 하였다. 해당 연구의 목적을 위해 장애인고용패널 2차 웨이브 중 5차 연도(2020년) 데이터를 이용하였으며, 이 중, 15세 이후 장애를 입게 된 신체장애인 2,264명을 분석 대상으로 하였다. 다중회귀분석을 통해 분석한 결과, 장애 기간에 따른 장애수용도의 변화가 유의미하지 않은 가운데, 중증 장애는 경증 장애 대비 장애수용도가 유의미하게 낮게 나타났다. 그러나, 장애 기간과 장애수용도의 관계에, 장애 정도의 조절 효과를 검증하였을 때, 유의미한 연관성을 보였다. 즉, 중증 장애인의 경우 장애 기간이 늘어남에 따라 장애수용도가 증가한 반면, 경증 장애인은 오히려 장애수용도가 소폭 감소하는 것으로 나타났다. 이는 중증과 경증 장애인 사이에 인구통계학적, 사회경제적, 건강상태 등의 요인 이외에 심리적 기제가 차별적으로 작용하기 때문으로 판단된다. 이러한 결과는, 정신건강의 증진을 위한 심리적 개입의 방향에서 중증 장애인과 경증 장애인 각각 차별적인 접근이 필요함을 시사하고 있다.

영문 초록

The present study aims to explore the influences of the length of disability and the degree of disability severity on the acceptance of disability among persons with a physical disability. Drawing on the stage theory of grief, we hypothesize the association between the length of disability and the acceptance of disability as well as the moderating role of the degree of disability severity (i.e., mild vs. severe) in facilitating the acceptance of disability after controlling for demographic information, socioeconomic factors, and subjective health satisfaction. Using a sample of 2,264 individuals with a physical disability earned after the age of 15 in the 2nd wave of the PSED (Panel Survey of Employment for the Disabled) collected in 2020, our multiple regression analysis shows that the direct effect of the length of disability on the level of acceptance of disability is not significant while the severity of the disability is significantly associated with the level of acceptance of disability. In terms of the interaction effect of the degree of severity between the length of disability and the level of acceptance of disability is significant, which means that the level of acceptance of disability increases as the length of disability increases among severely impaired disabled but not among mildly impaired disables. These findings support the argument of the stage theory of grief claiming that people who experience critical loss tend to go through the stage of psychological changes in the direction of accepting their loss instead of staying in acute emotion. Our findings suggest the importance of the differential influence of psychological factors such as the perceived degree of disability severity in the mental health of disabled people. Theoretical and practical implications are discussed.
