교정시설 내 수용자 자살예방 대책

국문 초록

이 연구목적은 교정시설 내 수용자 자살의 원인을 생물심리사회적 요인으로 나누어 분석하고 이를 공중보건모델에 따라 예방대책을 수립해 봄으로써 교정시설 내 수용자 자살예방정책을 수립하는데 도움이 되고자 하는데 있다. 이러한 연구목적을 달성하기 위하여 우선 교정통계연보와 최근의 교정시설 내 자살 수용자를 대상으로 한 심리부검 결과를 기초로 이루어진 연구 등 각종 문헌을 중심으로 자살의 원인과 특징을 살펴보았다. 그 결과, 교정시설 내 수용자 또한 일반인과 마찬가지로 다양하고 복잡한 원인에 의해 자살생각, 자살계획, 자살행동으로 진행되고 있음을 확인할 수 있었다. 이에 따라 교정시설 내 수용자 자살예방 대책을 공중보건모델에 근거하여 다음과 같이 제시하였다. 1차 예방은 일반 수용자를 대상으로 하는 사전관리예방(prevention)으로 체계적인 조기발견 및 평가체계 구축 등을 제시하였다. 2차 예방은 자살 위험성이 높은 수용자들을 대상으로 하는 위기개입예방(intervention)으로 전문상담 등 위기개입 강화를 위한 사회적 유대 강화를 강조하였다. 3차 예방은 자해 및 자살시도자를 대상으로 하는 사후관리예방(postvention)으로 자살사고 대응 응급조치 및 긴급후송 체계 구축 등을 제안하였다. 이러한 제안이 원활하게 작동되기 위해서는 무엇보다도 해당 인력의 충분한 지원과 전문화가 우선 되어야 할 것이다.

영문 초록

The purpose of this study is to analyze the causes of prisoners suicide in correctional facilities by bio-psychological factors and establish preventive measures in accordance with the public health model to help establish policies for the prevention of prisoners suicide in correctional facilities. In order to achieve this research objective, the causes and characteristics of suicide were first examined focusing on various literature, such as a correctional statistical yearbook and the study based on the results of a recent psychological autopsy on suicide prisoners in correctional facilities. As a result, it has been confirmed that prisoners in correctional facilities, like the general public, also undergo suicide thoughts, suicide plans and suicide actions due to various and complicated causes. Accordingly, the measures to prevent suicide of prisoners in correctional facilities were proposed based on the public health model as follows: The primary prevention is pre-management prevention for general prisoners, proposed systematic early detection, and evaluation system construction. The secondary prevention is an intervention for prisoners with high suicide risk, and emphasizes strengthening social ties to strengthen crisis interventions such as professional counseling. The tertiary prevention is post-management prevention for self-harm and suicide attempters, and suggested emergency measures to respond to suicide accidents and the establishment of an emergency evacuation system. In order for these proposals to work smoothly, sufficient support and specialization of the relevant personnel should be prioritized above all.
