Predicting Vocational Rehabilitation Outcomes among Older Stroke Survivors

이 연구는 상대적으로 경험적 연구가 부족한 뇌졸증 병력이 있는 환자들의 직업재활이란 측면에서, 2009년 미국 장애인직업재활서비스 부서에서 수집된 55세 이상 뇌졸증을 경험한 적이 있는 샘플군을 기반으로 직업재활에 영향을 미치는 요인들에 중점을 둔 연구이다. 샘플은 크게 성공적으로 재활된 사례부터 재활되지 않은 사례들을 포함하는 690명의 피험자들을 CHAID분석을 통해 이들 샘플의 성공적인 직업재활을 가장 잘 예측할 수 있는 요인을 추출하는 방식으로 진행되었다. 연구결과, 재활보조공학기구 이용여부가 통합고용으로의 성공적인 취업을 가장 잘 예측하는 요인으로 판별되었으며, 나이, 성별, 공적부조, 직업배치서비스 역시 뇌졸증을 경험한 장애인들의 직업재활에 중요한 영향을 미치는 요인들로 판별되었다. 이러한 발견은 미국의 재활서비스 부서에서 수집된 방대한 자료들을 각각의 장애집단별로 직업재활 성공여부를 예측하는데 필요한 요인들을 확인할 수 있는 계기를 마련하며, 또한 뇌졸증을 경험한 장애인들의 직업재활의 성공을 위해 개인적 장애관련 편차를 고려해 어떠한 직업재활 서비스들이 연계되어야 하는지에 대한 방향을 제시한다.

영문 초록

There is limited research in the vocational rehabilitation literature which examines employment issues among older stroke survivors. This study analyzed the Rehabilitation Services Administration case service report for fiscal year 2009 to explore demographic and vocational rehabilitation service (VR) variables impacting employment outcomes of persons age 55 and older who have experienced a stroke in the United States. The sample consisted of 690 people with stroke who were closed either a) rehabilitated or b) not rehabilitated by state/federal vocational rehabilitation agencies. Exhaustive chi-squared automatic interaction detector (CHAID) analysis was used to extract predictive information for employment outcomes of older individuals recovering from stroke. The CHAID analysis indicated that receiving rehabilitation technology services increased the probability of competitive employment for the target population. Age, gender, public supports, and job placement services were also significant factors in predicting employment outcomes among older individuals who have experienced a stroke. Together, these findings suggest certain demographic characteristics and VR services interact to predict the likelihood of employment outcomes among the target population. Clinical implications and suggested research directions are provided to improve employment services and increase employment rates among older adults who have experienced a stroke.
