일본 지역포괄케어시스템의 고찰과 시사

국문 초록

인구고령화는 장기요양보호를 필요로 하는 노인층의 급증과 노인의료비 증가 및 국가재정 압박으로 다가온다. 이것은 선진국의 공통된 경험이다. 특히 일본의 급속한 고령화는 의료 및 복지에 대한 수요를 증가시키고, 이에 따른 정부의 정책적 방안들이 제도화되고 있다. 일본정부가 추진하는 지역포괄케어시스템은 2003년 설치된 고령자개호연구회에서 ‘존엄을 유지하는 케어를 목표로 향후 생활의 계속성을 유지하는 새로운 케어서비스체계’로서 처음 등장하였다. 본 연구는 이러한 지역포괄케어시스템을 신자유주의정책의 관점에서 다음과 같이 정리하고 있다. 첫째, 공적책임의 축소와 비공식적 자원의 동원강화이다. 둘째, 공급주체 다양화를 통한 영리영역의 확대 및 공급주체 간 경쟁원리의 강화이다. 일본정부가 추진하고 있는 ‘정책모델의 지역포괄케어시스템’이 아닌 언제든 어디서든 누구나 비용 걱정 없이 공적책임 강화의 필요성을 제기한다. 또한 사회보장에 대한 국고부담의 확충, 필요한 의료가 제공 될 수 있도록 의료보장제도의 충실, 의료 및 개호종사자, 행정, 주민의 연대 등을 통해 누구나 자택에서 존엄한 생활을 영위 할 수 있는 ‘실천모델 지역포괄케어시스템’의 구축이 필요함을 제시한다. 이러한 일본의 사례를 통해 알 수 있듯이 한국의 커뮤니티 케어의 구축은 새로운 실버비즈니스 모델이 아닌 공적책임이 강화된 복지적 관점에서 만들어져야 한다는 점을 시사하고 있다.

영문 초록

Population growth of elderly causes fast growth in need of long-term nursery care and expense growth for medical budgets, also a burden on national finance. This is a common case in developed countries. Especially rapid population growth in Japan has made more demand on medical and social welfare needs that was systemized by governmental policies. The community total care system implemented by Japanese government appeared as “the new care system that maintains sustainability of living set as a purpose of maintaining dignity” from the Study Group for Elderly Care in 2003. This study focuses on summarizing the community total care system with a view of Neoliberalism’s policy. First, it is minimization of public responsibility and enhancement of unofficial resources. Second is enlargement of profitable area through diversification of supply subjects and enforcement of competitive cause between supply subjects. It is suggested that in need of enhancement of public responsibilities without worrying of cost anytime and anywhere, not the ‘community total care system’ implemented by Japanese government. It is also suggested that enforcement of ‘implement model for community total care system’ that anyone can maintain their dignity life at home through enlargement of governmental budget for social welfares and faithful implementation of medical welfares and joint efforts between medical and care professionals, administration, citizens. As it can be recognized by the case of Japan, the community care implementation needs to be established not in a silver business model but in a view of social welfare that is exemplified by public responsibilities.
