호스피스 환자의 불안과 삶의 질에 대한 노래회상 음악치료

국문 초록

본 연구의 목적은 노래회상 음악치료가 호스피스 환자의 불안과 삶의 질에 어떠한 영향을 미치는지 알아보는 것이다. 연구 참여자는 B시와 Y시에 소재한 병동형 호스피스 기관에 입원한 40세 이상의 말기암 환자 총 3명이다. 이를 위해 노래회상 음악치료 중재 전·후 불안검사(STAI)와 삶의 질 검사를 실시하여 점수 차이를 분석하였다. 또한, 회기별 긍정적 혹은 부정적 언어와 비언어적 반응을 분석하였고, 삶의 질 척도의 하위영역인 신체적, 심리·정서적, 사회적, 영적 서술들을 범주화하였다. 연구 결과, 노래회상 음악치료는 호스피스환자의 불안을 감소시키고, 삶의 질 하위범주 가운데 심리·정서적, 사회적, 영적 범주의 점수를 향상시키는 것으로 나타났다. 또한, 질적 분석을 통해 언어적, 비언어적 반응에서 긍정성은 증가하는 반면, 부정성은 감소하는 것으로 확인되었다. 특히, 노래회상 음악치료가 환자들의 심리적 안정감 증가와 불안감소를 도움으로써, 그들에게 영적 지지를 제공하고, 결과적으로 삶의 질을 향상하는 것으로 밝혀졌다. 이는 노래회상 음악치료가 호스피스 환자의 심리·정서적 안정감을 증진할 뿐 아니라 총체적 삶의 질 향상에도 긍정적인 영향을 줄 수 있다는 것을 시사한다.

영문 초록

The purpose of this study is to investigate the influence of singing reminiscence music therapy on hospice patient s anxiety and the quality of life. The participants of the study were three patients who are hospitalized in cities B and Y, aged over forty with terminal cancers. For this study, the researchers conducted the State-Trait Anxiety Inventory (STAI) test and the quality of life scale. The scores before and after singing reminiscence music therapy of both were compared. In addition, positive or negative narratives and non-verbal reactions per session were analyzed, and physical, emotional/psychological, social, and spiritual narratives were also categorized which are subcategories of the quality of life scale. The results showed that singing reminiscence music therapy palliated hospice patients’ anxiety. It also indicated that the scores of emotional and psychological, social, and spiritual subcategories of the quality of life were increased. Furthermore, positivity of verbal and non-verbal reactions increased while negativity of them decreased through the qualitative analysis. Especially, singing reminiscence music therapy provided the patients with spiritual supports and improved quality of life because it increased psychological stability of the patients and decreased anxiety. This implies that singing reminiscence music therapy can promote hospice patient’s psychological and emotional stability as well as can lead the improvement of patients’ overall quality of life.
