각막굴절교정렌즈와 저농도 아트로핀 안약이 안축장의 변화에 미치는 영향 비교

국문 초록

목적: 각막굴절교정렌즈와 저농도 아트로핀 안약의 안구 성장 억제효과에 대해 비교 분석하였다. 대상과 방법: 각막굴절교정렌즈와 0.025% 저농도 아트로핀 안약 치료 시작 시 성별과 나이가 같고 2년 이상 경과관찰이 가능했던어린이에서 치료 전후의 안축장검사 결과를 분석하였다. 결과: 총 74명(92안)이 연구에 포함되었으며, 평균 나이는 9.5 ± 1.3세였다. 치료 전 구면렌즈대응치는 각막굴절교정렌즈군 -2.50 ± 0.83 D, 저농도 아트로핀 안약군 -2.51 ± 0.80 D였으며, 안축장은 각막굴절교정렌즈군 24.44 ± 0.91 mm, 저농도 아트로핀 안약군24.67 ± 0.80 mm였다. 치료 시작 시 두 군 간 구면렌즈대응치 및 안축장은 통계적으로 유의한 차이가 없었다. 각막굴절교정렌즈군에서 렌즈 착용 2년 후 안축장이 24.96 ± 0.90 mm로, 안축장 변화량은 0.53 ± 0.30 mm였다. 저농도 아트로핀 안약군에서 치료 2년후 안축장은 25.16 ± 0.77 mm로, 안축장 변화량은 0.49 ± 0.26 mm였다. 치료 2년 후 두 군 간 안축장 및 안축장 변화량의 차이는통계적으로 유의하지 않았다. 또한, 두 군 모두에서 치료 시작 연령과 안축장 변화량 사이에 유의한 음의 상관관계가 나타났다. 결론: 각막굴절교정렌즈와 0.025% 저농도 아트로핀 안약 치료군 간 2년간의 안축장 성장 속도에는 유의한 차이가 없었다. 그러나두 치료군 모두 어린 나이에서 안구 성장이 더 빠르게 진행하는 경향이 관찰되었기에, 조기에 근시가 발생한 경우 적극적인 치료고려가 중요하다.

영문 초록

Purpose: To compare the effects on axial elongation and associated factors between orthokeratology lenses and 0.025% low-concentration atropine eye drops. Methods: A retrospective analysis was conducted on the medical records of children matched by sex and age, with a spherical equivalent difference within 0.125 diopters, who were followed for more than 2 years after starting treatment with orthokeratology lenses or 0.025% low-concentration atropine eye drops. The results of refractive error and axial length were analyzed using an independent t-test, paired t-test, Pearson correlation test, and multiple regression analysis. Results: Seventy-four patients (92 eyes) were included in this study. Before treatment, the axial length was 24.44 ± 0.91 mm in the orthokeratology lenses group and 24.67 ± 0.80 mm in the low-concentration atropine eye drops group. There were no statistically significant differences between the two groups in age at the start of treatment, sex, spherical equivalent, and axial length before treatment. At the 2-year follow-up, the mean axial length change was 0.53 ± 0.30 mm in the orthokeratology lenses group and 0.49 ± 0.26 mm in the low-concentration atropine eye drops group, respectively. There were no statistically significant differences between the two groups in the mean axial length change. Additionally, the age at the start of treatment showed a significant negative correlation with the axial length change in both the orthokeratology lenses group and the low-concentration atropine eye drops group (p = 0.001, p = 0.011, respectively). Conclusions: There was no significant difference in axial elongation for 2 years between orthokeratology lenses and 0.025% low-concentration atropine eye drops treatment groups. Since younger age was associated with faster eye growth, it is important to actively consider treatment in cases of early onset myopia.
