초고령사회와 치매: 고베프로젝트를 통해 본 지역사회 기반 통합 관리 방안

국문 초록

목적: 2 025년 초고령사회로의 진입이 예상되는 우리나라는 베이비붐 세대의 고령화로 노인 인구 비율이 급증하고있으며, 이는 치매 인구 증가로 이어질 것으로 전망된다. 일본 고베시는 치매 예방과 조기개입을 목표로 ‘고베프로젝트’를 통해 치매 친화적 지역사회를 조성하기 위한 다양한 연구와 정책을 추진하였다. 이에 본 연구는 초고령사회의 사회적 부담을 경감시킨 고베프로젝트를 통해 한국 초고령사회의 과제를 논의하고 대응책을 모색하고자 한다. 방법: 본 연구에서는 지역사회 기반 의 치매관리방안을모색하고자 한국과 일본의 고령화 추세와 지원 정책에 관한사례를 검토하였다. 또한 관련 전문가들과의 자문회의를통해 지방자치단체 단위에서의 치매관리 방안에 대한 제언을 도출하였다. 결과: 일본 고베시는 WHO와 고베대학과 협력하여 약8만명의 70대 이상 고령자를 대상으로 ‘고베프로젝트’를 진행하였다. 고베프로젝트는 첫째, 고령자의 인지저하 여부를 평가하여 돌봄 위험의 관계성을 규명하고, 둘째, 치매예방교육과 인지훈련을 포함한 치매 고위험군에 대한 인지저하 개입의 효과성을 탐색하며, 셋째, 치매조기발견과 개입을 실현하는 지역 모델을 제시하고 넷째, 기존 사회보장영역 내에서 해결되지 않는 치매환자가 일으킨 사고에 대한사고구제제도 방안을 제시하였다. 이러한 고베프로젝트는치매 관리의 모범 사례로 평가받으며, 일본 내 다른 지역과전 세계적으로 정책적 모델로 활용되고 있다. 결론: 고베프로젝트는 치매 환자와 피해자를 모두 보호하는 지역사회 기반의 통합 관리 모델로 제도적 안전망을강화하고 치매 친화적 환경을 구축한 모범적 사례이다. 치매는 더 이상 개인과 가족의 문제가 아니며, 국가와 지방자치단체, 사회 구성원의 협력을 통해 효과적인 대응책을 마련해야 할 시점이다.

영문 초록

In Korea, which is expected to enter a super-aged society by 2025, the proportion of the elderly population is rapidly increasing owing to the aging of baby boomers, which is expected to lead to an increase in the dementia population. In Japan, Kobe City promoted various studies and policies to create a dementia-friendly community through the ‘Kobe Project,’ with the aims of dementia prevention and early intervention. Therefore, this study aims to discuss the tasks of the super-aged society in Korea and seek countermeasures through the Kobe Project, which alleviated the social burden of the super-aged society. Korea provides long-term and customized care services for the elderly in need of care and is strengthening medical and care services by introducing the National Dementia Responsibility System in 2017. However, policies for relieving accidents and victims caused by dementia are insufficient. Kobe City, Japan, in cooperation with the World Health Organization (WHO) and Kobe University, conducted the ‘Kobe Project’ for approximately 80,000 elderly individuals in their 70s or older. The Kobe Project first examines the relationship between caring risks by evaluating whether the elderly have cognitive decline. Second, it explores the effectiveness of cognitive decline interventions for high-risk groups of dementia, including dementia prevention education and cognitive training. Third, the Kobe Project presents a regional model that realizes early detection and intervention for dementia. Fourth, it suggests an accident relief system plan for accidents caused by dementia patients that are not resolved within the existing social security area. The Kobe Project is evaluated as an exemplary case of dementia management and is used as a policy model in other regions of Japan and around the world. The Kobe Project is an example of strengthening the institutional safety net and establishing a dementia-friendly environment with an integrated community-based management model that protects both patients with dementia and victims. Dementia is no longer a problem only for individuals and families; it is time to develop effective countermeasures through cooperation between the state, local governments, and members of society.
