중증정신질환자가 이용하는 지역사회 정신건강서비스에 대한 체계적 고찰

국문 초록

목적 : 이 연구의 목적은 지역사회 정신건강서비스가 중증정신질환자에게 미치는 영향과 체계적인 문헌 고찰을 통해 중증정신질환자에게 서비스의 중요성을 제시하고, 효과에 대한 근거를 마련하고자 하였다. 연구방법 : 2013년 이후부터 2023년까지 데이터 베이스 RISS, GOOGLE SCHOLAR, PubMed에 주요 검색어를‘중증정신질환자 AND 정신건강서비스’,‘중증정신장애인 AND 지역사회서비스’,‘Severe mental illness AND Mental health services’, ‘Severe psychiatric disorders AND Community Service’를 사용하여 선정기준과 배제기준을 바탕으로 논문을 검토한 결과, 최종적으로 13편의 논문을 연구 분석 후 선정하였다. 결과 : 연구의 질적수준은 총 13편으로 5단계 분류 방법에서 Level Ⅰ 4편(30.7%), Level Ⅱ 3편(23.4%) Level Ⅳ 2편(15.2%), Level Ⅴ 4편(30.7%)로 분류되었다. 중재 대상자는 조현병과 양극성 장애가 포함된 9편(69.2%)으로 가장 많았으며, 서비스 종류에는 동료지지 제공서비스, 지역사회 참여와 계획 서비스, 주거제공서비스, 지역사회 정신건강서비스, 인지강화치료와 적극적인 지역사회치료가 제공되었다. 그 결과 삶의 질, 대인관계 향상, 우울 증상 감소, 정신건강 의료 이용률과 가족옹호 및 연대 영역에서 유의미한 효과가 나타났다. 결론 : 향후 연구에서는 질적 수준이 높은 근거 확보를 위해 다양한 진단을 받은 정신질환자를 대상으로 한 비교연구가 진행되어야 할 것으로 사료된다.

영문 초록

Objective : The purpose of this study was to present the impact of community mental health services on patients with severe mental illness, the importance of services to patients with severe mental illness through a systematic literature review and provide evidence for their effectiveness. Methods : Literature published between 2013 to 2023, 13 studies were selected for analysis based on inclusion criteria and exclusion criteria using the main search terms 'Severely mentally AND Mentally disabled OR Psychosis AND Mental health services AND Community service AND Service' in the databases RISS, PubMed, GOOGLE SCHOLAR. Results : The quality level of the research is a total of 13 papers, which are classified into LevelⅠ 4 papers(30.7%), Level Ⅱ 3 papers(23.4%), Level Ⅳ 2 papers(15.2%), and Level Ⅴ 4 papers(30.7%) in a 5-level classification method. It has been done. The largest number of intervention targets included schizophrenia and bipolar disorder 9(69.2%) and service types included peer support services, community participation, planning services, housing services, community mental health services, cognitive enhancement therapy, active treatment and community treatment was provided. As a result, significant effects were found in the areas of quality of life, improvement of interpersonal relationships, reduction of depressive symptoms, mental health care utilization, and family advocacy and solidarity. Conclusion : In future research, it is believed that comparative studies should be conducted targeting people with mental illness who have received various diagnoses in order to secure high-quality evidence.
