뇌졸중 클라이언트에게 적용한 작업수행에 기반을 둔 작업치료에 대한 국내 연구 동향 분석: 주제범위 문헌고찰

국문 초록

목적 : 본 연구는 국내에서 보고된 뇌졸중 클라이언트에게 적용한 작업수행에 기반을 둔 작업치료에 대해 분석하고, 이에 대한 적절한 연구 방향을 제시하는 것이 목적이다. 연구방법 : 구글 학술검색(Google scholar), 국회도서관(www.nanet.go.kr), 한국교육학술정보원(RISS), 한국학술정보(KISS)를 통해 2014년 1월부터 2023년 12월까지 보고된 연구를 검색한 결과, 총 26편의 연구가 선정되었다. 그리고 작업수행에 기반을 둔 작업치료로 클라이언트 중심(client-centered) 치료가 12편, 작업기반(occupation-based) 치료를 적용한 연구가 14편이었다. 각 연구에 대해 게재년도, 표본 수, 연구 디자인, 작업수행에 기반을 둔 작업치료의 방법, 종속변수, 치료에 따른 결과, 연구의 제한점으로 나누어 분석하였다. 결과 : 측정 변수들은 뇌졸중 회복 정도를 포함하여, 작업수행, 작업참여, 일상생활활동, 삶의 질, 삶의 만족도, 상지기능, 인지기능, 편측무시, 균형, 우울, 스트레스와 자기효능감을 포함한 심리사회적 요인에 대한 것이었으며, 작업수행, 작업참여, 일상생활활동, 삶의 질 등에서 긍정적인 결과를 나타냈다. 결론 : 본 연구는 뇌졸중 클라이언트를 위한 치료 방법으로 작업수행에 기반을 둔 작업치료의 연구 방향성을 제시했으며, 뿐만 아니라 연구의 질적인 향상을 도모했다는 것에 의의가 있다.

영문 초록

Objective : The purpose of this study is to analyze occupational therapy based on occupational performance applied to stroke clients who are reported in Korea, and to suggest appropriate research directions. Methods : As a result of the research search, a total of 26 studies were selected. Additionally, there were 12 studies that adapted client-centered intervention and 14 studies that adapted occupation-based intervention as occupational therapy based on occupational performance. Each study was analyzed by occupational therapy method, dependent variable, treatment results, and study limitations based on by publication year, the number of samples, study design, occupational performance. Results : Measured variables were about a number of psychosocial factors including stress, self-efficacy, work performance, work participation, ADL, quality of life, life satisfaction, upper extremity function, cognitive function, neglect, balance and depression as well as the degree of stroke recovery, and they had mostly positive effects. Conclusion : This study has a significant meaning that it suggests research directions for occupational therapy based on occupational performance as a treatment method for stroke clients, while also seeking to improve the quality of research.
