충동조절장애의 개관과 임상적 특성에 대한 검토

국문 초록

질병에 대한 분류, 특히 정신장애에 대한 진단이나 분류는 지난 70여 년 동안 괄목할만한 성장과 뚜렷한 성과를 보이고 있다. 그러나 충동성 조절의 어려움은 본인뿐만 아니라 주변 사람들에게도 심각한 고통을 주거나 해로운 결과를 초래하기도 한다. 이 연구에서는 충동조절장애 관련 하위 유형에 대해, DSM과 ICD 체계를 혼합해서 충동조절장애의 부류에 속하는 하위 유형 중에서 간헐적 폭발장애, 병적 도박, 병적 도벽, 발모광, 병적 방화에 대해 개관하고, 각각 장애의 임상 특성에 대해 알아보고자 하였다. 충동조절장애 관련 연구들을 탐색한 결과, 충동조절장애에 대한 약리학적 치료 방법을 연구하는 데 상당한 진전이 있었지만 대부분의 연구 표본이 높은 위약 반응률, 그리고 제한된 추적 기간을 가지고 있기 때문에, 이 연구들의 일반화의 가능성은 제한적이었다. 충동조절장애에 가장 효과적인 치료법을 찾기 위해서는 추가적인 약리학적 치료 방법을 비롯한 여러 방법에 대한 연구가 필요하다. 또한 어떤 치료법이 누구에게 효과적인지 정확하게 평가하기 위해서는 유망한 치료 연구에 대한 반복 연구와 약물치료와 기타 치료법을 비교하는 추가 임상 시험이 필요하겠다. 이 연구들을 통해 얻은 지식은 충동조절장애 치료를 발전시킬 뿐만 아니라 충동조절장애를 가진 사람들과 그 질환에 직간접적으로 영향을 받는 사람들의 삶을 상당히 개선시킬 것이다.

영문 초록

The classification of diseases, especially the diagnosis or classification of mental disorders, has shown remarkable growth and remarkable results over the past 70 years. However, difficulty in controlling impulsivity can cause serious pain or harm not only to oneself but also to those around you. In this study, for the subtypes related to impulse control disorder, the DSM and ICD system were mixed to provide an overview of intermittent explosive disorder, pathological gambling, pathological kleptomania, trichotillomania, and pathological pyromania among the subtypes belonging to the category of impulse control disorder, and to investigate the clinical characteristics of each disorder. Exploring the studies related to impulse control disorder, there has been significant progress in studying pharmacological treatments for impulse control disorder, but the possibility of generalization of these studies is limited because most of the study samples have a high placebo response rate and a limited follow-up period. In order to find the most effective treatment for impulse control disorder, studies on several methods, including additional pharmacological treatments, are needed. In addition, replication of promising treatment studies and additional clinical trials comparing drug treatment with other treatments are needed to accurately assess which treatment is effective for whom. The knowledge gained through these studies will not only advance the treatment of impulse control disorder, but also significantly improve the lives of people with impulse control disorder and those directly or indirectly affected by the disorder.
